have personally inspected the above referenced property in the City of Fort Collins, Colorado, and verify
that construction of all stormwater facilities and grading there is complete, and that the elevations and
drainage patterns shown on the attached drawing(s) which were developed from a survey performed on
(Date) 'I-~-0 ~ , accurately and truly depict the elevations and drainage patterns as
they existed on the date of the survey. Based on this information, I certify that (check one):
j The elevations and drainage patterns of the lot are in substantial conformance with the City
approved drainage and grading plan or revised grading plan for the property (information

o The elevations and drainage patterns of the lot as measured are in general conformance with
the City approved drainage plan or revised grading plan with exceptions (information attached).

Re: Drainage Certification for Eye Center Optical of Northern Colorado LLC, Parking Lot Add ition
Dear Mr. Barber ,
We recently surveyed grading , drainage, and related landscape improvements associated with the parking
lot addition at the Eye Center Optical of Northern Colorado , LLC. The bu ilding and improvements are
located on the west of Riverside Avenue and south of Prospect Road in Fort Collins. Spot elevations were
checked at pertinent locations and compared to the grading plan to verify whether the grading met the
intent of the approved grad ing and drainage plan . The enclosed drawing shows both the grading plan and
as-bu ilt spot elevations.
In general , the parking lot construct ion meets the requ irements of the grading and drainage p lan and there
is positive dra inage away from the building that flows into a water quality swale adjacent to Riverside
Avenue . If you have any questions, please call.
