Dear Mr. Farinelli
Staff has reviewed the Mountain View Chiropractic Clinic PUD Prelimi-
nary/Final plan and has the following comments to make:
y A letter indicating that there are no additional paved or impervious areas
VVV proposed with this development needs to be signed and dated by the prop-
erty owner and submitted to the Planning Department.
2. As indicated at the January 2 Conceptual Review meeting, staff does not
view the addition of a support staff employee as creating additional
impact on the site; however, the addition of an associate, creating an
expansion in the existing business, is of concern. As stated in my letter of
January 3 to you, a letter addressing the current business operation (ie..
hours of operation, number of clients seen, number of employees, amount
of parking currently used, any increase of business due to the additional
employee, etc.) must be submitted. The letter submitted with the develop-
ment application does not provide any of this information and does not
indicate the responsibilities of the additional employee.
The 150' information needs to be shown on the same page as the site plan.
This information needs to indicate building locations/type of use and
/ parking lot/driveway locations.
4! Information regarding the business also needs to be placed on the site plan.
1� Needed is the current number of employees, proposed additional employee
responsibilities (ie. support staff).
Clarification is needed on the site plan regarding the amount of existing
clinic space, as well as the amount of residential space.
Signage needs to be addressed. If an existing sign will be retained, a note
should so indicate on the site plan.
V The existing parking spaces need to be delineated on the site plan. The
typical dimension is 8' x 23'.

08s A vicinity map must also be shown on the site plan, rather than as a
separate page.
The following information also needs to be placed on the site plan:.

ratio of floor area to lot size, broken down by use (ie. residence
-area is % of overall site);
u number and location of parking spaces;
aximum building height;
xisting zoning;

od Planning and Zoning Board signature block.
By Wednesday. April .4. 1590 three (3) copies of the revised site plan and any
other information needed to address these comments must be submitted to the
Planning Office. By Monday. Aoril_30— 1990, ten (10) folded sets of the site
plan, a colored rendering (unfolded) of the site plan and an 8-1/2" x 11" PMT
reduction must be submitted for the Board meeting May 7. If you have any
