To Whom It May Concern:
AT&T Wireless is proposing to install telecommunications equipment on the structure identified above. Federal
Communications Commission (FCC) rules allow AT&T Wireless to install the equipment without the delays
associated with a historic preservation review if the equipment will not adversely impact the environment. Your
answers to the inquiries below will allow AT&T to determine whether the equipment might cause an impact. Please
mark your response to these questions:
