Please fill in the blanks with names and titles exactly as they should appear and return this form. Hard copy or email to your development review engineer is fine. This form is for informational purposes only and is not your development agreement. It requires no signatures. If you should have any questions please call your development review engineer. This information should match that shown on a plat (if applicable).
