Provide a vicinity map on the annexation plat. Show existing structures and street names. 3. Add the following signature block to the annexation plat: APPROVED AS TO FORM: City Engineer 4. The petition needs a notarized "Cjrculator Signature". 5. Please provide a statement as to why annexation and zoning is being requested. 6. The Parks and Recreation Department would like to meet with the owner and/or developer to discuss the possibility of a neighborhood park sonic - where in the Clarendon Hills master plan area.
