Dear Don:
Staff has reviewed the South Shields Veterinary Clinic PUD Preliminary Plan
and has the following comments to make:
1. Water service will be provided by the Fort Collins -Loveland Water District
and sewer by the South Fort Collins Sanitation District. You are respon-
sible for working directly with the districts regarding service for this site
and meeting any necessary requirements the districts may have.
L/2: The access easement that straddles Lots 1 and 2 needs to be dedicated as
an access and utility easement on the final plat. This easement should be
labelled for both access and utility purposes on the preliminary site plan.
A minimum 24' wide hard surface as far west as the parking area
entrance is required with this plan. The remainder of the drive to the
west must be a minimum width of 20'. Since this access would also serve
as access for future uses on Lot 3, the long-term access needs must be
addressed. This could be accomplished by a future dedication at the time
Lot 3 is developed.
3/ For fire access purposes, an 80' diameter turnaround is necessary. The
turnaround may be provided on lot 3 if an easement is provided and may
be constructed of road base.
Lam, A fire hydrant capable of providing 1000 gpm (gallons per minute) at 20
psi residual pressure must be provided on the west side of Shields, within
400' of the proposed structure.
5. Employee parking spaces do not have adequate back-up depth, unless the
entire 30' access easement is paved. The intent for paving and proposed
pavement width in this area of the access easement is not clear on the site
plan. The parking area in front of the clinic must be paved and have
wheel stops or curbing provided and the driveway must also be hard -
surfaced.
6. A street light is planned at the northeastern corner of the site.

`7. The following information needs to be added and/or clarified on the site
plan -
a. Title plan as "South Shields Veterinary Clinic, Preliminary PUD Site and
Landscape Plan".

existing landscaping on Lots I and 3 also needs to be shown.

9: Information regarding the proposed sign size and materials must be sub-
mitted.
DLO. Screening must be provided along the north side of the proposed clinic, to
buffer from the adjacent residence to the north and along the rear (west)
property line of Lot 2 to screen from future residential uses. Since
Shields Street is an arterial street, street trees, planted on 40' centers,
should be provided on the landscape plan along the site's Shields Street
frontage.
11. The future intent for Lot 3 must be clarified, in terms of density. Since
there is no intent to develop Lot 3 at the same time as the rest of the
site, a master plan will be required. Please refer to the attached
requirements for a master plan. The master plan would be a separate plan
from the preliminary submitted.
12. The following items must be submitted by Friday. September 29 in order to
retain this item on the October 23 Planning and Zoning Board agenda:
- preliminary storm drainage report and plan;
- preliminary soils report;
- traffic impact study; and
- preliminary utility plans.
1�! A 15' utility easement adjoining the Shields Street right-of-way must be
shown on the site plan and dedicated with the final plat.
�1�4! The driveway entrance at Shields and the length as far west as the parking
area entrance should be 24' wide, so that two vehicles may easily pass each
other.
15. A 15' radius is needed for the driveway cut on Shields Street.

17'. It appears that the site does not meet the minimum required points on the
Business Service Point Chart (for vet clinic) or the Residential Density
Chart (minimum density of 3DU/acre). Further clarification of this will
be possible when the 150' information is provided on the site plan. If
point charts are not met, a variance from the chart(s) must be requested
and supporting rationale for a variance must be submitted.
By noon on Wednesday. October 4. 1989, five (5) copies of the revised site plan
addressing these comments must be submitted to this office. By Monday,
October 16, 1989 ten (10) folded sets of the site plan, a colored rendering
(unfolded) and an 8-1/2" x 11" PMT reduction must be submitted.
If you have any questions or would like to meet with
comments, please do not hesitate to contact me. I will
October 5 - October 16 and therefore, would suggest that
these items as soon as possible.

The procedure to be followed in processing a master Ulan is as indicated in the
section of this manual entitled "Review Process." The purpose of this checklist
is two -fold: (1) To itemize for the applicant all information and documents
required for full and complete review; and (2) To provide a list for checking
completeness of applications as they are submitted. Section 29-526 of the Code
of the City of Fort Collins (Land Development Guidance System) should also
be referred to for a specific listing of submittal requirements. The following
information and data are required:
A. Application form (one copy) and filing fee ($60.00).
B. Twenty-five (25) copies of the master plan at an appropriate scale, with an
outer dimension of 24" x 36" (folded to fit into a legal size folder with the
development name showing) must be submitted containing the following infor-
mation:
(1) Parcel size, in gross and net acres.
(2) Existing topographical character of the land at a contour
appropriate with the scale of the project, showing all water
courses, floodplains, unique natural features, significant wild-
life areas, and vegetative cover, including trees and shrubs.
(3) Existing zoning.
(4) Maximum height of all structures.
(5) Approximate acreage and density (gross) of each area; num-
ber, and type of residential units; and estimated floor area and
types of business, commercial and industrial uses.
(6) Location and general nature of each land use.
(7) Total land area and approximate location and amount of open
space in each land use.
(8) Approximate location of proposed and existing major streets
and major pedestrian and bicycle routes, including major points
of access.
(9) Approximate location and size in acres of any public use
proposed such as parks, school sites, and similar public or semi-
public uses.
(10) Area shown on the master plan must extend beyond the prop-
erty lines of the proposal to include a survey of the area within 150
feet of the proposal, exclusive of public right-of-way, at the same
scale as the proposal and including the following:
(a) Land uses, location of principal structures and major existing
landscape features;
(b) Densities of residential uses;
(c) Traffic circulation system;
(d) Natural features of the landscape; and
(e) General topographical mapping at same scale as master plan.
(11) A vicinity map of the area surrounding the site within a
distance of at least one mile, showing the following:
(a) Zoning districts;
(b) Traffic circulation system;
(c) Major public facilities; and
(d) Location of existing municipal boundary lines.
(12) Owners' certification of acceptance of conditions and
restrictions as set forth on the master plan.
(13) Signature block for Secretary of the Planning and Zoning
Board's certification of approval of master plan.
(14) Attorney's or owner's certification of ownership.
C. The master plan must be accompanied by:
(1) The name and address of each owner of property in the Master
Plan area.
(2) A current list of the names and addresses of all owners of
record of real property within 500 feet of the property lines of
the parcel of land for which the master plan is proposed, exclu-
sive of public right-of-way. (This information is available from
the Assessor's Office in the Larimer County Courthouse or from
local title companies.) This list must be typed on 8-1/2" X 11" sheet -
type mailing labels (33 labels per sheet) by the applicant.
(3) A statement of planning objectives (25 copies) including:
a. Statement of appropriate City Land Use Policies achieved by pro-
posed master plan.
b. Statement of standards of open space, buffering, landscaping and
circulation.
c. Estimate of number of employees for commercial and industrial
uses.
d. Description of rationale behind the assumptions and choices made by
the applicant.
(4) A legal description of the site on 8-1/2" X 11" sheet (2
copies).
(5) A statement of proposed ownership of public and private open
space areas (25 copies).
(6) A development phasing schedule (25 copies).
(7) A conceptual drainage plan (2 copies).
(8) Other documentation as determined by special circumstances,
including a Traffic Impact Analysis (see section of this manual
entitled "Traffic Impact Analysis Guidelines").
(9) One rendered set (unfolded) of master plan and ten (10) full
size copies (folded), to be submitted one week prior to the Plan-
ning and Zoning Board meeting.
(10) One 8-1/2" x 11" PMT reduction or comparable quality (xerox
copies not acceptable) of master plan to be submitted one week prior
to the Planning and Zoning Board meeting.
(11) Signed reproducible mylar of master plan to be submitted one
week prior to the Planning and Zoning Board meeting.
(12) A list of names of all general and limited partners and/or
officers and directors of the corporation involved as either
applicants or owners of the master plan.
