Description of project: This is a request to install cellular equipment on the roof of the Lemay Avenue
Health & Rehab Facility at 4824 S Lemay Ave (parcel# 8606276001). The proposed equipment would
be mounted on the roof at the north end of the building and would project 9 feet above the existing
parapet. The equipment would be screened by a 10 foot wall. The proposed project is within the
Harmony Corridor (HC) zone district and is subject to Administrative (Type 1) review.

comments offered informally by staff during the Conceptual Review will assist you in preparing the
detailed components of the project application. Modifications and additions to these comments may be

As of January 1, 2015 all development plans are required to be on the NAVD88 vertical
datum. Please make your consultants aware of this, prior to any surveying and/or

The design of this site must conform to the drainage basin design of the McClellends Creek
Master Drainage Plan as well as the Fort Collins Stormwater Criteria Manual.

If there are no site improvements that require grading or an increase in impervious area,
there are no Stormwater requirements. Please contact Water Utilities Engineering if site
improvements are anticipated.

The 2017 city wide Stormwater development fee (PIF) is $8,217/acre for new impervious
area over 350 square feet and there is a $1,045/acre of site review fee. No fee is charged for
existing impervious area. These fees are to be paid at the time each building permit is
issued. Information on fees can be found at:

fees. There is also an erosion control escrow required before the Development Construction

accordance with the Fort Collins Stormwater Manual.

Planning Services
1. When submitting project drawings please include photo simulations after the title sheet. -Included
Please provide explanation of why the wireless equipment cannot be located as far away from
the buildings edge as possible.Because of the existing mechanical equipment (which causes feedback
and interference) and the dimensions of this building we placed our antennas as close to the middle of
the building as we were able to get without comprising the functionality of the site.

Please provide screen material specification sheet as part of plan set. This should include
thickness, material composition and texture. Staff will require a material sample prior to
hearing. Screen design must be consistent with the existing exterior materials, roof form,
scale, mass, color, texture and character. -Added in

Any conduit or equipment mounted to the exterior of the building must be painted to match.-Noted in

Can all rooftop antennas and screening be located inside the existing parapet wall? -Please see note (2)
above, due to interference with HVAC and other rooftop equipment Antenna needs to be placed away from central parapet.

8. If the proposed fiber vault is above ground it must be screened by landscaping and irrigated.
The proposed development project is subject to a Type 1 review and public hearing, the
decision maker for Type 1 hearings is an Administrative Hearing Officer. The applicant for
this development request is not required to hold a neighborhood meeting for a Type 1
hearing, but if you would like to have one to notify your neighbors of the proposal, please let
me know and I can help you in setting a date, time and location for a meeting. Neighborhood
Meetings are a great way to get public feedback and avoid potential hiccups that may occur
later in the review process.

This development proposal will be subject to all applicable standards of the Fort Collins Land
Use Code (LUC), including Article 3 General Development Standards. The entire LUC is

If this proposal is unable to satisfy any of the requirements set forth in the LUC, a Modification
of Standard Request will need to be submitted with your formal development proposal.
