Dear Bob,
I will outline for you the information regarding transportation for 1500 Casa Grande that was
requested in the letter dated 12/20/96 (Martin Luther Home - Group Home Review #33-96).
The anticipated vehicle trips to and from the property can best be outlined per day as there is no
hourly schedule to and from the property. Martin Luther Home staff will be providing
transportation for the residents of the home in an agency vehicle. The residents all work at the
same location and will be transported to and from work each day. Other trips would include
typical residential activities such as grocery shopping, leisure activities and medical appointments.
An average day would call for 2-3 trips to and from the home for the residents.
Staff will be arriving and leaving the house to work their shifts. The times of the shifts may vary,
but are typically aftemoon/evening and overnight on weekdays and aftemoon/evening, overnight
and daytime on weekends. There will be one staff on duty the majority of the time with a second
staff present for specific activities, training and additional coverage if needed.
