We offer assistance with homemaking and activities. Our residents will not require a locked facility, but will be involved members of our community. We anticipate our home will be an added feature to the quality of the community in which it is located. While we will be providing 24 hour supervision, we do not anticipate more than two employees on the premises at any one time, and therefore we will have adequate off street parking. Our visitors will be parking on the street, as we are located on the corner of a cul-de-sac. All of our staff will have a background check conducted by the Colorado Bureau of Investigation to ensure the safety of the community. We will be licensed in accordance with the regulations as established by the State Department of Health for a Personal Care and Board Home as well as those for an Alternative Care Facility. The facility has four bedrooms, which will provide housing for 7 residents. We will provide affordable housing for two residents by obtaining approval as an Alternative Care Facility with Medicaid reimbursement. We will have personnel on premises at all times, but not more than two employees at any one time. The driveway of the facility offers two off-street parking places, which should be adequate to provide for our employees. We have made arrangements with the Unitarian Church to use their parking lot for any additional parking needs. We estimate there will be no more than 5 - 6 trips to and from the facility daily to accommodate residents and employee needs. Our residents will not be bringing automobiles with them to this facility. The breezeway contains a handicap accessible ramp, which is accessed from the driveway to accommodate ADA requirements. We will not be making any changes to the exterior of the existing structure on the property. We will be hanging a sign from the overhang of the house which will say "Yorktown Corner" and not be more than 1.5' x 2'. This facility will not be closer than one mile to the nearest senior group home.
