The petition must contain the following information: (a) ✓ Name and address of applicant. (b) ✓Name and address of owner of subject property. (c) /Legal description of property to be rezoned (if amending the zoning map) plus a map of the area to be rezoned: (d) Existing text of Land Use Code and proposed language amendments using strikethrough (for proposed deletions) and underline (for proposed additions) format (if text amendment is proposed). (e) / Requested zoning classification and any conditions to be voluntarily placed upon the rezoning (if amending the zoning map). (2) J Written justification of rezoning request addressing the review criteria contained in Section 2.9.4(H) of the Land Use Code (original and 5 copies). (3) ✓Application form (1 copy) and filing fee. (4) ,/Three lists of names and addresses of all owners of record of real property within five hundred (500') feet (see Supplemental Notice Requirements of Section 2.2.6 of the Land Use Code for further information) of the property lines of the parcel of land for which the overall development plan is proposed, exclusive of public right-of- way. Two lists shall be typed on mailing labels (33 labels per . sheet), the other list shall be a reproducible copy of those labels.
