The enclosed area will need to fully landscaped and screened from view from the adjacent street and any
future pedestrian or bicycle trails. Additional landscaping may be needed around the perimeter of the
enclosure.

Lake Park Drive reconstructed in 1998. Cutting of this street will incur substantial street cut fees. Recommend
boring street to install needed utilities. If street is cut, then I need to see a plan stating so and showing limits of
repair area for street cuts

Transformer on Lakeshore Circle is not sized to serve a wireless tower. A new transformer will need to be
installed near the tower site. Sprint to provide ROW or easement for the new transformer. Contact Light and
Power immediately to coordinate new transformer location.

Will need a Commercial Service Form (C-1) to show load requirements.

Meter must be placed outside of fence or visible through fence for meter reading
