[3/24/05] The parking stalls along the north lot line are only allowed to be
reduced to a 17' depth if they overhang a 6' wide walk or landscape strip
(Sec. 3.2.2(L)(4) of the LUC). The plan indicates that the width of the
landscape strip is only 5% therefore the parking stalls need to be 19' in
depth.
A variance request is being submitted with revisions.

[3/30/05] Parking lot and drives require min of 1.Ofc Location of BB fixture
at south is blocked by BH Spruce - move fixture away from plant massing.
Similar issue with south BB on west side - relocate north. Light fixtures at
west entrances should provide 1.Ofc. Recommend fixture on west side
where facility sidewalk meets sidewalk along Lemay. Recommend lighting
along internal walkways on north, south, and west.
Landscaping has been adjusted to maintain separation distances.

Topic: Off -site easements
Number: 37 Created: 3/29/2005
[3/29/05] The construction of ST-1 will involve acquiring an off -site
easement from the owners of Tract A of Oakridge, if the area within Tract A
where the work is proposed is already a drainage easement, then approval
from the owner of that property should be secured.
A drainage easement has been worked out with Everitt Companies to allow for
the construction of the off -site storm sewer. A reception # is not available at this
time, but the easement has been shown on our Plat and throughout the plan set.
Topic: Plat
Number: 26 Created: 3/28/2005
[3/28/05] Please add a drainage easement for all areas encompassed within
the proposed detention ponds.
Private Drainage Easements have been added to Plat and are shown in the plan
set.

[3/18/05] The variance request for driveway separations on Lemay needs to
be rewritten - it needs to address Oak Ridge to the north, and the church
driveway to the northwest, which is very close to this proposed drive cut.
Safety, health, and welfare of the public needs to be addressed as well, and
the request needs to be stamped. It might be helpful to review LCUASS
1.9.4 for the items needed in a variance request.
It is the City's intent to grant the variance once the revised request is
received.
So noted. Reference revised variance request.
Number: 27 Created: 3/29/2005
[3/29/05] It appears that a segment of the proposed stormwater line offsite
(northeast corner) is not within an existing easement. Please provide a
letter of intent from that property owner prior to scheduling a hearing, and
the completed deed must be submitted prior to mylars.
A drainage easement has been worked out with Everitt Companies to allow for
the construction of the off -site storm sewer. A reception # is not available at this
time, but the easement has been shown on our Plat and throughout the plan set.

Topic: Grading
Number: 13 Created: 3/18/2005
[3/18/05] The grades in the ROW and across the sidewalk appear to be too
steep to meet ADA for the sidewalk (2% cross -slope) and the slope
requirements for 2' each side of walk (2%). The maximum slope in the
ROW is 4:1. Anything above that would require a retaining wall, which
would require an engineering variance, which would not be looked upon
favorably.
2' each side of walk has been re -graded to 2%. Sidewalk and ROW grades have
been checked.
Number: 16 Created: 3/18/2005
[3/18/05] Offsite grade lines should be shown for at least 50' offsite.
Offsite contours have been added to the 50' requirement.
Number: 5 Created: 3/18/2005
[3/18/05] The proposed 10' utility easement on Lemay still needs to be
approved by XCEL.
A letter from XCEL will be submitted acknowledging their approval.
Number: 6 Created: 3/18/2005
[3/18/05] The portion of the sidewalk on Rule that is located outside of the
ROW needs to be placed in a public access easement on the plat.
A public access easement has been added to the Plat.
Number: 7 Created: 3/18/2005
[3/18/05] Parking setbacks (LCUASS Ch. 19) need to be met from both Rule
and Lemay. On Lemay, the first parking stall should be no closer than 75
feet from the Lemay flowline. On Rule, the first parking stall should be no
closer than 50' from the Rule flowline.
Parking adjacent to Lemay Avenue and Rule Drive has been revised per
LCUASS Ch. 19 standards. Reference site plan.
Number: 8 Created: 3/18/2005
[3/18/05] On the site plan, please label existing ROW widths for Lemay and
Rule, and proposed parkway widths.
ROW and parkway widths have been added. Reference site plan.
Number: 18 Created: 3/18/2005
[3/18/05] The standard radius for the driveway onto Lemay is 20% not 25'.
A variance request is being submitted with revisions.
Number: 19 Created: 3/18/2005
[3/18/05] Driveways should be concrete to ROW line. If the concrete is
extended beyond that, an expansion joint should be provided at the ROW
line.
Concrete has been moved to ROW line.
Number: 21 Created: 3/18/2005
[3/18/05] Please see redlines and utility plan checklist for any additional
comments.
So noted.
Number: 14 Created: 3/18/2005
[3/18/05] Please use the standard under -walk drain from the pond onto
Rule - straight chase (no bends) and only 2' wide unless otherwise
approved by Stormwater. Also, the metal covering should extend from 2'
behind the walk out to the curb, per the detail.
Sidewalk culverts were changed to D10's and called out as such in the Grading
and Concrete plans. A D10 detail was added to Detail sheet 14.
Number: 15 Created: 3/18/2005
[3/18/05] The offsite work shown northeast of the project - are there
existing easements to accommodate this? Please show/label/include the
reception number of these easements, or provide a letter of intent from the
adjacent property owners to dedicate needed easements.
A drainage easement has been worked out with Everitt Companies to allow for
the construction of the off -site storm sewer. A reception # is not available at this
time, but the easement has been shown on our Plat and throughout the plan set.
Topic: General
Number: 2 Created: 3/18/2005
[3/18/05] The sidewalk along Lemay should be located at the back of the
ROW, as opposed to meandering in the middle of the parkway. Also, the
new street trees should be located in the ultimate parkway, approximately
5' from the edge of the walk when it is placed against the ROW.
A variance request is being submitted with revisions.
Number: 3 Created: 3/18/2005
[3/18/05] All plans need to meet the scannability requirements in LCUASS
appendix E. Questions about scanability can be directed to JR or Tim in
Technical Services.
So noted.
Number: 4 Created: 3/18/2005
[3/18/05] The driveway onto Rule Drive should be a standard cut, not
radius style.
A variance request is being submitted with revisions.

Response to staff Project Review comments
Dear Mr. Shepard,
We have reviewed the staff letter dated April 05, 2005 and have the following
comments: Please note the site plan and building plan have been completely
redesigned since conceptual review.
ITEM: 131-Bed Nursing Home at 4824 South Lemay Avenue (Rule
Property — northeast corner of Lemay Avenue and Rule Drive)
COMMENTS:
Department: Engineering Issue Contact: Katie Moore
Topic: Drainage
Number: 12 Created: 3/18/2005
[3/18/05] Approximately 25% of the site is shown to drain to the driveway
to Rule and over the public sidewalk onto the street. This is not allowed.
The water must be diverted, perhaps to an under -walk drain, so that it does
not cross the walk. Does this water need to be treated?
A 10' grated inlet, trickle pan and Di sidewalk culvert were added to the
southeast corner of the site to direct runoff to Rule Drive and not cross over the
sidewalk. The undetained area was included in calculations for the Water Quality
Control Volume in Detention Pond 3. This way, we are treating a greater volume
in Pond 3 to account for the undetained/untreated runoff that is being released
directly to Rule Drive.
