Hi Amy,
I found the dedication statement for one of two access easements in the submittal packet and upon
reviewing it, discovered that the City's standard dedication language has been altered. We will not accept
the terms and conditions paragraph that's been added and I'm rejecting this dedication statement. The
dedication statement needs to be completed using the UNALTERED template I've sent out already. I've
attached another copy in case you or the client doesn't have it anymore.
Please resubmit the correct dedication statement at your earliest convenience. You can do it before the
comments are due or I can enter the comment with this round and leave it as an open item for the next
round of review. We still need the dedication for the other access easement along the frontage road.

Hi Shelby,
Our client just sent over a legal description and exhibit to supplement the
deed of dedication for that 20' Access Easement at 4900. 1 don't know if
what we submitted was enough or who exactly needs this information. I'll
attach it to this email as a PDF, and if you guys need an original I can get
one from the surveyor.
