City staff has been told that the official title for this annexation is Mountain View School
Second Annexation. The map currently shows the name to be Mountain View — Polarius
Annexation. Be sure and return all red -lined plans if/when you re -submit.

[2/10/09] Paul Eckman, the Deputy City Attorney, indicated that Petitioner's/Owner's
Signature block on the 2nd page of the Petition needs to be dated December 12, 2008, to be
consistent with the date on Attachment "C" - Attorney Certification.

[2/10/09] Dan Kunis, the GIS Specialist for Larimer County, indicated that it is the
County's expectation that, as part of the annexation, the entire right-of-way of any County
roads adjacent to the property will also be annexed. In this case the only road adjacent to the
property is LaPorte Avenue, which has already been annexed in its entirety with the
Mountain View School Annexation and the Maxwell's First Annexation.
