CLC Responds: Due to cover constraints, a 19" x 30" elliptical reinforced concrete pipe will be
installed. Plans and report have been revised.

[11/17/09] On Sheet PDP-4, move the reference to 6-inch gate valve from Note 12 to Note 11
as shown on redlined plans.
CLC Responds: The 6-inch gate valve notation moved to note 1 1. The 8-inch gate valve
removed from drawing per conversation with Roger.
CLC Responds: This note was on the plans previously but was attached only to the utility
street cuts. When the utility street cuts went away so did the note. We will return the note to the
grading sheet and associate it with the proposed driveway cuts.

existing 9 ft. utility easement.

[11/19/09] Per the land use code "Drive aisles leading to main entrances shall have walkways
on both sides of the drive aisle". However, Sidewalks will not be required on the east side of the
internal private drive at this time. When parcels on the east side of the internal private drive are
developed, sidewalks will be required.

[11/13/09] Please call out the curb return radii at the three driveway entrances and station the
driveways on Custer and Illinois so that they can be located in the field. Please call out the
internal radii for the drives as well.
CLC Responds: The curb return radii have been on the site plan. We will add the curb return
radii (both internal and at drive entrances) to the grading plan within the utility set. We will also
put in stationing per the comment.

(11/13/09] Not 100% sure yet, but the offsite easement may only be dedicating an access
easement and not an access and emergency access easement. I'll know when I see the final
dedication statement. The city prefers to have the access and emergency access easement but
would accept just an access easement if it is not possible to obtain both. Anyway, the labelling .
for the offsite easement may have to be changed slightly but can be done quickly before mylars.

111/13/09] Will need the final dedication papers for the offsite easement plus the TDRF fee of
$250 and the County filing fees (TBD) prior to the final plan app.ovals. I will also send the
development agreement information sheet over so that we can start on that as well.

[11/17/09] Please add spots at the hp of the driveway entrances. The hp should be placed at
such a manner that no more than 750 sf of drainage should drain into the row without a chase.
CLC Responds: The only entrance that has a high point is the driveway to the church parking
lot: The high point was added to insure adequate cover over the pipe. The high point cannot be
.moved to the constraint listed because that would not provide cover over the pipe. If the point
was high enough to provide the cover for the pipe and be closer to the exit then slopes to
existing would exceed allowable limits. A cross pan is being placed and the drainage (although
greater than 750 sf it is still insignificant) will outfall to the same inlet that the drainage ditch
outfalls to.

Limits of street cut are approximate. Final limits are to be determined .in the field by the City

11/13/09] There are a lot of construction related notes on the Site Plan that are more
appropriate for the Utility Plan set and really should be removed from the Site Plan.
CLC Responds: Notes regarding signage, type of curb etc are typical to site plans (industry
standard) and would rather leave them on the site plan. The construction notes have been on
the site plan since the first submittal in February.

[11/13/09] Please correct line 47 (remove "(list)" and replace with "NA". Please correct line 48
to read something like as follows: LCUASS Table 8-2 requires 20' curb return radii for high
volume driveways off Local and Collector streets. A variance was granted to allow 25' curb
return radii.
[7/14/091 Missing the .General and Construction Notes (only some of them are shown on the
cover sheet).
[4/7/09] You missed quite a few items that are required on the plan sets before going to public
hearing and labeled them instead as NA. This is not correct. Please go through Appendix E4
and address all highlighted items that are not shown or shown incorrectly on your plan set.
Please call if you have questions so we can avoid unneccessary rounds of review.
[2/24/09] Please see LCUASS for all utility plan requirements and go through and complete all
highlighted items in Checklist E-4 with the next round of review. All highlighted items must be
designed in the plan sets before the project may go to hearing. All other applicable items must
be completed in Final Compliance and prior to final plan approval.
