Property Use
• Cooper Home Program provides transition services to approximately 15-22
students each school year. The facility is used Monday — Friday from
approximately 7:45am to 3:OOpm. (times may vary slightly due to
transportation availability).
• Because the program is community based, the students are not on site for the
full school day, but rather learning how to use public transportation, or
transported to internships, community activities and jobs in the white school
bus.
• Students attend generally 2.5 days per week — this result is 10 students or
fewer arriving at Cooper Home Tuesday - Friday. On Monday's all of the
students will attend.
• 4 Staff. Special Education teacher, (2) Para Educators and (1) Employment
Specialist.
• The PSD provides indoor janitorial services, and outdoor landscaping
services as well as snow removal.
• Vehicle traffic —1- PSD designated Cooper Home 14 passenger white school
bus arrives in the AM (7:45--8:15) to drop students off at Lesher Middle
School and return in the afternoon (2:45 —3:00 PM) to pick students up to
return home. Any student needing a wheelchair will be picked up and
dropped off at the 1309 Newsom address.
• The 4 staff members each have a personal vehicle.
• In addition, the Cooper Home Program has a small white 14 passenger white
school bus that will be parked at nearby Lesher Middle School at certain
times.
• The program runs on the same calendar as the school district. If students
qualify for services over the summer, the building will be in use from the end
of August through the end of June. When school is not in session, the district
still maintains the grounds.
Entry to the Cooper Home Program
The Student...
• Must be between the ages of 18-21
• Is currently receiving services through the Integrated Services Program
in the Poudre School District
• Has "Transitional Needs" identified on their IEP (Individualized
Education Program)
• Is interested in interacting within the community
• Has earned high school credits required for graduation before entering
the program
Cooper Home Students...
• Are eligible for transportation through PSD Transportation Services
• Attend the program 3 days each week, but may vary depending on
student needs
• Pay $25 for a bus pass and $5 for a Parks & Recreation pass. In addition,
incidental money is needed for recreation and leisure activities
• Upon completion of the program, receive a Certificate of Completion or a
PSD Graduation diploma if graduation requirements have been met
Job Skills & Employment Opportunities
Students learn skills through participating in a variety of volunteer
opportunities throughout the community. Volunteer opportunities include
Poudre River Libraries, Habitat for Humanity, the Food Bank, HELP
International, Animal House, etc.
In addition, internships are available at Colorado State University including the
mail room and gardens, and through local businesses including PETCO, ARC,
Red Robin, Terracon, Taco Bell, etc.
Leisure and Recreation
Students participate in adaptive recreation activities throughout the community
including Ft Collins Parks and Recreation, EPIC and North side Aztlan Center,
and are exposed to Taekwondo through a local Martial Arts Center.
In addition to physical activities, students attend Open Stage performances at the
Lincoln Center and attend monthly evening activities.
Graduation & Ceremonies
Upon completion of the Cooper Home Program, students who have met PSD
graduation requirements while at high school will receive a PSD Graduation
Diploma through their school.
Also upon program completion, students have the opportunity to celebrate their
transition through a Send -Off Graduation from Cooper Home.
Community Services
Students have access to many resources: Colorado State University Nutrition
Center, CSU Occupational Therapy Department, the Food Bank, the Women's
Closet, ARC and many more.

Transition Services
Students in the Cooper Home program transition into their next phase of life by
gradually increasing responsibility in order to gain independence. Services students
receive include:
• Job Coaching
• Training in work -related skills
• Planning menus, grocery shopping and cooking
• Riding public transportation
• Developing organizational skills, including keeping a weekly schedule
• Planning recreation and leisure activities
Cooper Home Program Goals
Students will:
• Practice daily life skills
• Build social skills
• Become more mobile in the community
• Demonstrate community safety skills
• Access community resources
• Investigate employment opportunities
• Demonstrate self -advocacy
• Access leisure and recreational activities
Poudre School District [PSD] is currently looking for a replacement for the current
`Cooper Home' located at 740 E Stuart.
PSD has determined that the multi -level Stuart Street house is not able to meet the
varied program and ADA requirements of the students. PSD intends to sell the
current facility once the replacement facility has been purchased.
PSD currently has placed under contract a residential house located at 1309
Newsom St. This house was built in 1959, is a one story, frame, ranch style house
with -1350 square feet. The address is zoned R-L.

facility.
