Project This is a request for the 5th Minor Amendment to Harmony Technology Park
Description: ODP. This amendment will shift the primary uses to parcel G from parcel M in
exchange for secondary uses shifting to parcel M from parcel G. There will be
no net change In the acreage of primary or secondary uses. The planned
segment of Timberwood Drive between Lady Moon Drive and Cinquefoil Lane
will continue to provide connectivity, but as a private drive integrated into the
Banner Health Medical Center design -specific development plan, with an
emergency access easement overlay. Only parcels owned by Imago Enterprises
are affected by this change.

SUBMITTAL REQUIREMENTS:
1) Eight (8) copies of RECORDED plan (i.e. site, landscape, elevation, etc. - whichever is being altered) on 24" x
36" size (usually available at the Technical Services Department at 281 N. College) — RED -LINE THE
CHANGES ON the eight plans or provide eight copies of revised drawings showing the changes in addition to the
eight originals. ALL PLANS MUST BE FOLDED TO 8 1/2" X 11";

Detailed description of the change and reason(s) for the request: Shifting of primary uses to parcel G from parcel M in
exchange for secondary uses shifting to parcel M from parcel G. There will be no net change in the acreage of primary or secondary
uses. The planned segment of Timberwood Drive between Lady Moon Drive and Cinquefoil Lane will continue to provide connectivity,
but as a private drive integrated into the medical center design -specific development plan with an emergency access easement overlay.
