Dear Resident/Property Owner:
On Thursday, June 21, 2012, at 6:00 p.m., at City Hall, 300 LaPorte Avenue, Fort Collins,
Colorado, the Fort Collins Planning and Zoning Board will conduct a public hearing to
discuss a land use development proposal in your neighborhood. The project is referred to
as Elderhaus Office (1008 Mentz Place) - Addition of a Permitted Use.
The request consists of adding an office use to an existing 1,004 square foot single-family
residence located at 1008 Mantz Place. Currently, the subject property is located in the
Neighborhood Conservation Low Density (N-C-L) Zone District and "office uses" are not
included on the list of permitted uses in this zone district. If approved by the Planning and
Zoning Board, the proposed use would only be allowed for the existing (single-family
residence) and proposed (office) activities on the subject property and not on a zone
district -wide basis.
The proposed office is planned for the existing Elderhaus Adult Day Programs located
across the street at 605 South Shields Street. Four support staff employees will work from
8 a.m. — 5 p.m., Monday through Friday at this location. The Elderhaus is a missionary
program of the American Baptist Church, located directly to the north of the proposed
office.
The only physical changes proposed to the property are a new street tree in the front yard
and an alteration to the sidewalk in order to be, compliant with the American with
Disabilities Act (ADA).

guide may aid you in understanding your role in the development review process and how
to provide effective input at or before the public hearing.
Finally, the list of affected property owners for this neighborhood meeting Is derived from
official records of the Larimer County Assessor. Because of the lag time between home
occupancy and record keeping, or because of rental situations, a few affected property
owners may have been missed. Please feel free to notify your neighbor of this hearing so
all stakeholders may have the opportunity to attend. If you are unable to attend the public
hearing, but would like to provide input, written comments are welcome via U.S. mail to the
