06/07/2011: The applicant's response letter states that the pole will be designed to
collapse. A note to that effect needs to be added to the plan, probably on Sheet Z1.
[2/4/11] The 60' tall pole requires that the pole be setback 60' from the lot lines. If its
setback less than 60', then it needs to be designed to collapse rather than topple. Since
there's no overall site plan, I don't know what the setback is. If it's less than 60', then we'll
need plans or documentation explaining that it will collapse.
Be sure and return all of your redlined plans when you re -submit.

06/10/2011: It appears that on the new site plan several utility easement are shown along
with a access easement. Will these be dedicated via seperate document or will they be a
portion of the lease agreement with the property owner? If being dedicated to the City/Utility
provider by seperate document, please contact me for information on the process of
dedication and the information/fees associated with the process.
[2/22/11 ] This facility will require some sort of access easement/agreement so that
access to the facility is ensured. Is there an existing access easement for the existing
parking lot? If not a access easement will need to be dedicated via separate document
from the public street through the existing parking lot to the facility. If a easement is not
desired, language included in the lease agreement that ensures access to the facility will
also suffice.

[2/22/11] As noted in Comment #2, the setback of the pole must be equal to or larger than
its height. Under no circumstance may a pole fall into public ROW. Please either move the
pole so that it meets the setback criteria or demonstrate and document that the pole will
crumple rather than topple.

shall be painted to match as closely as possible the color and texture of the wall, building
or surrounding built enviornment. Muted colors, earth tones and subdued colors shall be
used.
- The color selected, "passive", does not match the adjacent medical building. Please
resubmit a color chip selecting a tan color.
