Our philosophy is to focus on the individual not the limitations of the disease. We provide a familiar, secure and home -like environment filled with caring staff, pets, music, gardens and alternative therapies. New Dawn admits only memory impaired residents age 50 and older with Alzheimer's disease or other forms of dementia. Our residents are elderly individuals who are consequently quiet and no longer drive; thus traffic, noise and required parking are minimal. Our homes are residential, attractive, secure and well -kept and our on -site general managers are trained to be good neighbors and involved in the local community. The community will have a full-time, RN Director of Health Services and other LPNs or RNs covering the other shifts and weekends providing nursing coverage around the clock, seven days per week. Three full-time, specially trained Activities Directors work closely with the residents and staff for exceptional programming. New Dawn staffing includes a one to four staff to resident ratio on the day shift; a one to 6 ratio on the evening shift and a one to 12 ratio or a minimum of one awake caregiver on the night shift in each building with an additional nurse that floats between the three buildings. Staffing is adjusted according to occupancy and need. All units are strictly rentals. New Dawn will assume all ownership and maintenance of structures and common areas. Construction is anticipated to begin shortly after all necessary approvals are received. The project will be developed in one phase with an approximately eight month construction timeline. September 13, 2006 City of Fort Collins — Current Planning 281 North College Avenue Fort Collins, Colorado 80521 RE: New Dawn Assisted Living - Project Development Plan To Whom It May Concern: On behalf of New Dawn Assisted Living, we present this application for a project development plan on a 2.5-acre site within Rigden Farm, located at the southwest corner of Limon Drive and Kansas Drive. The project will consist of a three building assisted living complex totaling approximately 30,000 building square feet. An application is currently under review to rezone the site to M-M-N (Medium Density Mixed -Use District) in which long-term care facilities are a permitted use. The PDP and associated rezoning are consistent with the policies and goals of the Fort Collins "City Plan". Policy HSG-1.5 "Special Needs Housing" recognizes the housing needs of special populations within the community. The proposed development will meet this identified need. The site does not fall into any sub -area plan. The proposed development will not place any undue burden on the natural environment, public utilities and infrastructure, the transportation system or surrounding development. The currently vacant site is part of the larger Ridgen Farm development, no conflicts between our development and existing wildlife or wetlands exist. This assisted living use is a very quiet, non-impactful neighbor and should fit nicely into the surrounding development. Pedestrian circulation is encouraged within the site via an extensive network of sidewalks that interconnects all buildings and common areas. The New Dawn model consists of three, single -story buildings on a single site with each building containing 16 private rooms with private bathrooms. The community will operate as a secured environment with locked egress and egress access doors and a secured outdoor area to prevent elopement and protect the safety and security of the residents. These small, intimate buildings present a more home -like environment that does not over -stimulate our residents..Staff knows each other, families and the residents intimately. The building contains approximately 50% common space to resident room. There is a dining room; kitchen which can be used with supervision by the residents; activities room; laundry room; quiet room; sun porch and entry lounge.
