OF THE COUNCIL OF THE CITY OF FORT COLLINS
APPROVING CERTAIN AGREEMENTS TO BE ENTERED INTO
WITH OTHER AGENCIES TO PROVIDE HUMAN SERVICES TO THE CITY
WHEREAS, heretofore, the City of Fort Collins has received from the
Federal Government certain revenue sharing funds, and
WHEREAS, the Council has determined to use a portion of said funds to
develop human service programs in the City, and
WHEREAS, the Council has determined that some federally matchable
funds shall be used to develop human service programs for which the Federal
Government has set aside matching funds, and
WHEREAS, after consideration of the program which the City Council
desires to implement, it has been determined that it is in the best in-
terests of the City to contract with other agencies for the implementation
of some of these programs, and
WHEREAS, the Council has appointed three of its Human Relations
Commission members to serve on the Larimer Human Resource Board, the
purpose of such Board being to scrten human service agency applications and
recommend a coordinated human service delivery budget for Fort Collins,
Loveland, and Larimer County, and
WHEREAS, such Board has held its meetings and provided such recommen-
dations, and
WHEREAS, contracts have been prepared as follows
ELIZABETH STONE RESOURCE CENTER
Services - Employment and Personal Growth 225 of 600 unemployed or
underem�p oyed applicants are employed and increase their employment income
by $500,000 serving 270 employers (money generated is being pro-rated )
Child care 300 of 500 families applying receive 19,200 hours of child
care assistance provided by 35 individual workers and 143 day care homes
generating $25,000 Consumer Credit 210 of 252 applicants will receive
counseling and pay $65,000 of outstanding debts to 175 local creditors
Total cost of this program to the City of Fort Collins shall be Seven
Hundred Ninety-Seven Dollars and 50/100 ($797 50) per month ($9,570 00
annually) through December 31, 1981

UNITED DAY CARE CENTER
Special Conditions - Training 33 of 33 applicants satisfactori-
ly complete the enter s on-the-job day care teacher training Supportive
Parent Program 308 of 508 parents of day rare children will receive
support and referral services and correct parenting problems Day Cate
340 of 650 applicants received 30,140 days of child care Infant Toddler
Care 40 of 40 applicants received 10,080 days of infant/toddler care
Total cost of this program to the City of Fort Collins shall be Eight
Hundred Thirty-One Dollars and 50/100 ($831 50) per month ($9,978 00
annually) through December 31 , 1981
Special Conditions - United Day Care Center shall coordinate the
infant/toddler program with Larimer County Social Services Department and
Elizabeth Stone Resource Center' s Child Care Referral Training Program and
coordinate transportation services with Care-A-Van
VOLUNTEERS CLEARING HOUSE
Services - Life Shills Training 174 of 200 applicants complete an
indivi -u- y designed curriculum to increase their ability to provide for
the material and nutritional needs of their families Total cost of this
program to the City of Fort Collins shall be Three Hundred Thirty-Three
Dollars Dollars and 33/100 ($333 33) per month ($3,999 96 annually) through
December 31, 1981
Special Conditions - None
LARIMER ASSOCIATION FOR RETARDED CITIZENS
Services - Respite Care 60 of 80 applicant families receive 9,914
hours o respite care from 60 trained volunteers Advocacy 60 of 80
applicants receive 8,299 hours of advocacy towards independent living from
60 volunteer advocates Total cost of this program to the City of Fort
Collins shall be One Hundred Twenty-Five Dollars ($125 00) per month
($1,500 00 annually) through December 31, 1981
Special Conditions - Larimer Association for Retarded Citizens
shall consolidate information referral services Frith Community Crisis and
Information Center and share location and support services with the Larimer
Association of Blind and Physically Handicapped or justify not doing
so
BATTERED WOMEN'S TASK FORCE
Services - Safe House 840 of 940 applicants receive emergency

housinq and appropriate referrals Total cost of this program to the City
of Fort Collins shall be Three Hundred Forty Dollars and 42/100 ($340 42)
per month ($4,085 04 annually) through December 31, 1981
Special Conditions - Admission to house shall be prioritized on
the basis of funding areas or else reciprocal agreements worked out with
other houses in other areas
COMMUNITY CRISIS AND INFORMATION CENTER
Services - Comprehensive Information and Referral 5,852 of 6,160
applicants' information and referral needs are met Crisis Intervention
2,070 of 2,179 applicants' crises are reduced to non-crisis level Train-
ing 1,000 of 1,000 applicants receive educational presentations in re
crisis intervention , information/referral on specific topics such as
suicide Total cost of this program to the City of Fort Collins shall be
Two Thousand Five Hundred Seventy-Four Dollars and 58/100 ($2,574 58) per
month ($30,894 96 annually) through December 31, 1981
Special Conditions - Community Crisis and Information Center shall
reopen negotiations with Loveland "Help Line" to merge to increase the
effectiveness and efficiency of crisis intervention services in Larimer
County, maintain its services to Loveland residents , and consolidate
information and referral services with the Larirner County Association for
Retarded Citizens and the Blind and Physically Handicapped
LARIMER COUNTY YOUTH HOMES
Services - Loveland Emancipation Training Center 18 of 26 applicants
graduate to a homelike setting with no crime recidivism for one year
Prospect Street Evaluation/Assessment Center 22 of 30 applicants are
evaluated for placement recommendation and placed in a homelike setting
(natural family, foster home, group home, or treatment facility) within 90
days Mathews Street Family Treatment Center 19 of 27 applicants receive
family treatment and are placed in a homelike setting with no crime recidi-
vism for one year Total cost of this program to the City of Fort Collins
shall be Four Hundred Twenty-Six Dollars and 67/100 ($426 67) per month
($5,120 04 annually) through December 31 , 1981
Special Conditions - Larimer County Youth Homes shall redirect its
programs to provide quality services at reasonable cost to Larimer County
youths in a more cost effective way
HOME HELPS
Services - Homemaker Health Aid 300 of 325 applicants with health

problems receive home health care which sustains their ability to maintain
themselves in their homes and avoid institutionalization Total cost of
this program to the City of Fort Collins shall be One Thousand Five Hundred
Fourteen Dollars and 25/100 ($1,514 25) per month ($18,171 00 annually)
through December 31, 1981
Special Conditions - Home Helps shall consolidate services with
Visiting Nurses Asso at ion and Hospice or justify not doing so
LARIMER COUNTY ALCOHOL SERVICES
Services - Outpatient Alcohol Treatment 700 of 850 applicants
overcome their problems for six months Total cost of this program
to the City of Fort Collins shall be Five Hundred Fourteen Dollars and
67/100 ( $514 67) per month ($6,176 04 annually) through December 31 ,
1981
Special Conditions - Larimer County Alcohol Services shall (1)
provide cost information on their services compared to other like services
around this State, (2) divide programs and budgets among alcoholic out
patient services, education, and the Driving While Intoxicated Program, and
(3) explore employment counsel in cooperation with area employment counsel-
ing agencies
LARIMER COUNTY ASSOCIATION OF THE BLIND AND
PHYSICALLY HANDICAPPED ---�`
Services - 36 of 50 apolicants are transported via Cara-A-Van for a
total oT--2-0-0 -trips to and from the Monday night City recreation program for
the handicapped, monthly meetings of the Association, one cultural event
per month, and five handicapped representatives are transported to monthly
meetings of the City Handicapped Advisory Committee Total cost of this
program to the City of Fort Collins shall be Nine Hundred Ninety-Seven
Dollars ($997 00) annually through December 31, 1981
Special Conditions - Larimer County Association of the Blind and
Physically Handicapped shall consolidate information and referral services
with Community Crisis and Information Center and share location and support
services with the. Larimer- County A�,socration for retarded citizen-s ..
SENIOR CHUCK WAGON -
Services - Congregate Meals 2,000 of 2,000 applicants, 60 years and
over, maintain nutritional health Total cost of this program to the City
of Fort Collins shall be Eight Hundred Ninety-Seven Dollars ($897 00)
annually

Special Conditions - Senior Chuck Wagon shall agree to allocate the
City o or o ins appropriation to the purchase of transportation
services to its clients
NEIGHBOR-TO-NEIGHBOR
Services - Housing Counseling 2,208 of 5,000 applicants are assisted
and acqu'i e and/or maintain decent, safe and sanitary affordable housing
Total cost of this program to the City of Fort Collins shall be Five
Hundred Dollars ($500 00) per month ($6,000 00 annually) through December
31, 1981
Special Conditions - None
POUDRE-THOMPSON TRANSPORTATION CORPS , INC
Services - 900 of 900 applicants (60 years or older) are placed in
volunteer assignments, giving 114,000 hours of volunteer service to agen-
cies in Larimer County Total cost of this program to the City of Fort
Collins shall be One Thousand Sixty-Four Dollars ($1 ,064 00) annually
Special Conditions - Retired Senior Volunteer Program shall seek
financia support rom Estes Part.
PLANNED PARENTHOOD
Services - Contraceptive Medical Services 2,400 of 2,400 applicants
receive contraceptive materials and related medical services, education,
and counseling Total cost of this program the City of Fort Collins shall
be One Thousand Dollars ($1,000 00) annually
Special Conditions - None
SUNSHINE SCHOOL
Services - Child Care 70 of 140 applicants receive 7,500 days of
child care Total cost of this program to the City of Fort Collins shall
be One Hundred Fifty-Six Dollars and 25/100 ($156 25), pet month ($1,875 00--
annually) through December- 31, 1981
Special Conditions - None
WHEREAS, said agreements have been approved by the City Manager and
the City Attorney and the City Council desires to approve such agreements
and authorize their execution

NOW, THERFFOPE, BE IT RESOLVED BY THE COUNCIL OF THE CITY OF FORT
COLLINS that the terms and conditions of the agr eenents above referred to
be, and the same hereby are, accepted and approved
BE IT FURTHER RESOLVFD that the funding for all such contracts shall
be from revenue sharing funds
BE IT FURTHER RESOLVED that the City Manager and City Clerk be, and
they hereby are, authorized and directed to execute said agreements for and
on behalf of the City of Fort Collins
