As requested, during the period of March 12 through March 24, 2018, Earth Engineering
Consultants, LLC (EEC) personnel provided construction observation and testing services for the
referenced project. The services provided by EEC personnel during the referenced period were
on a part-time/on-call as-needed basis and included periodic site observations during various
stages of the concrete masonry unit (CMU) wall construction and non-destructive testing
services as described herein. The results of the field and laboratory testing services completed
by EEC personnel during the referenced period are included with this report.
Concrete Masonry Unit (CMU) Wall Construction Observations
During the referenced period, EEC personnel observed portions of the reinforcing steel and other
structural related elements during the 8-inch by 16-inch standard block CMU wall construction
for the stairwell core. Layout of the CMU wall alignments were provided by others. During the
daily site visits, (March 12, 2018), EEC personnel observed CMU wall construction at the
various wall alignments and recorded approximate gridline locations, elevations/courses for each
day, along with the observed horizontal and vertical reinforcement requirements and grout
placement as needed.
In general, for the portions of the CMU wall observed by EEC personnel, the vertical
reinforcement consisted of one (1) #5 rebar spaced 48 inches on-center, along with additional #5

rebar posited at each corner, end of wall and each side of openings. The horizontal
reinforcement consisted of 9-gauge “ladder-type” reinforcement spaced every 16 inches on-
center full-height of each CMU wall. At the various bond beam locations, below and above
windows, and at openings, EEC personnel noted the reinforcement consisted of one (1)
continuous #5 or two (2) continuous #4 rebar within the fully-grouted cell alignments, as
required. The horizontal and vertical reinforcement for the portions of the CMU walls observed
by EEC personnel during the referenced timeframe on a periodic basis as noted herein appeared
to be in general accordance with details presented on the structural drawings prepared for the
project by KL&A, Inc. dated November 3, 2017. During the grouting operations, EEC personnel
observed grout placement within the low lifts. Lap lengths, where applicable, were checked and
appeared to meet the minimum required lengths.
Non-Destructive Testing (NDT) Services
During the referenced period, EEC’s in-house certified ICC bolt and weld *inspector conducted
site visits on March 21 and 29, 2018 to perform visual observations of the structural framing
located in the approximate area of Grid Lines 1 through 5 from A through B.5 including areas
west of Grid Line A and radius north of Grid Line 1. Included are bolted column to beam
connections, bolted beam to beam connections, bolted beam to masonry connections, welded
radius HSS beam to angle connections, welded angle to column connections, welded plate to
radius HSS connections, welded plate to column connections, welded plate to bearing plate
connections and welded bearing plate to bottom beam flange connections. The connections that
were visually observed in the area of the referenced Grid Lines appeared to be in general
accordance with the project plans and details dated January 19, 2018 and SSK-3 diagram dated
March 12, 2018.
In addition, visual observations of the light gauge structural framing located in the approximate
area of Grid Lines 1 through 5 from A through B.5 including areas west of Grid Line A and
radius north of Grid Line 1. Included are top and bottom stud track connections, screwed top and
bottom track to stud connections and screwed header and jamb connections. The framing that
was visually observed in the area of the referenced Grid Lines appeared to be in general
accordance with the project plans and details dated January 19, 2018.

construction at the stairwell core wall alignments and recorded approximate gridline locations,
elevations/courses for each day, along with the observed horizontal and vertical reinforcement
requirements and grout placement as needed.

During the CMU construction phase for the project, EEC personnel tested/evaluated the grout
material used within portions of the wall alignments, as identified herein. Grout compressive
strength cylinders, (approximately 4-inch diameter by 7-5/8 inches in length), were molded on-
site in general accordance with ASTM specifications. The grout cylinders were molded on-site
within CMU blocks used to construct the CMU walls, and were extracted/cored using a 4-inch
inside diameter diamond bit core barrel upon retrieval and delivery to our Windsor laboratory 2
days following the initial preparation of the specimens. Results of the field and 7-day laboratory
compressive strength tests performed during the referenced period are provided herein while the
28-day compressive strength test results will be provided as they become available.
Testing of Cast-in-Place Concrete
EEC personnel observed and tested Portland cement concrete delivered to the site for placement
as indicated on the enclosed daily report for the referenced project, (please refer to Set No. 3),
which was inadvertently left out of our previous Submittal No. 2. Field testing performed on the
plastic concrete included slump, air content, unit weight, and concrete temperature. The ambient
air temperature at the time of placement was also recorded. Five (4-inch diameter by 8 inches in
height) concrete test cylinders were cast from a representative sample on the respective date for
laboratory curing and compressive strength testing. Results of the field and 7-day laboratory
compressive strength test results performed during the referenced period are included herein,
while the 28-day test results will be provided as they become available.

As requested, during the period of January 15 through February 10, 2018, Earth Engineering
Consultants, LLC (EEC) personnel provided on-site construction observation and testing
services for the referenced project. The services provided by EEC personnel during the
referenced period were on a part-time/as-needed basis and included field density testing of
structural fill materials, observations of reinforcing steel placed within the foundation footings
and stem walls and testing of cast-in-place concrete as described herein. The results of the field
and laboratory testing services completed by EEC personnel during the referenced period are
included with this report.
Field Density Testing Services
During the referenced period, EEC personnel performed observations and field density tests on
structural fill materials used to develop slab subgrades that had been placed/prepared and
compacted prior to and/or during our site visits. The field density tests were completed at
random locations within the areas tested and were performed in general accordance with ASTM
Specification D6938. Results of the field density tests completed during the referenced period
are shown on the attached summary sheet and approximate locations are “plotted” on the
attached site diagrams included with this report. The field density test results met the
compaction requirement of at least 95% of standard Proctor maximum dry density (ASTM
D698).

Reinforcement Observations
During the referenced period, EEC personnel conducted site visits on January 17 and 23 and
February 9, 2018 to perform visual observations of the reinforcing steel placed for footings, stem
walls, and the slab-on-grade at locations as identified on the enclosed “color-coded/highlighted”
site diagrams. The reinforcement for the continuous spread footings consisted of three (3) #4
continuous horizontal rebar per details 1/S200, 6/S200, 7/S200, 9/S200, and 12/S200 on the
structural drawings. The reinforcement for the footing pads consisted of either four (4) #5
horizontal rebar each way for the bottom mat or seven (7) #5 horizontal rebar each way for the
bottom mat per the isolated footing schedule on sheet S300. The reinforcement for the stem
walls consisted of #4 vertical dowels spaced at 16 inches on center and #4 continuous horizontal
rebar spaced at 12 inches on center per details 1/S200, 6/S200, 7/S200, 9/S200, and 12/S200 on
the structural drawings. The reinforcement for the slab on grade consisted of WWF 6X6 –
W2.1XW2.1 mesh with three (3) #5 continuous horizontal rebar in the longitudinal direction and
#5 continuous horizontal rebar spaced at 16 inches on center at the thickened portions in the slab,
as well as #4 vertical dowels spaced at 16 inches on center at slab pour overs per the note on
sheet 1/S101 and details 4/S200 and 6/200.
Clearances and lap lengths, where applicable, were checked and appeared to meet the minimum
requirements. The reinforcement for the footings, stem walls, and slab-on-grade as noted herein
and observed by EEC personnel on the dates indicated, appeared to be in general accordance
with details presented on the structural drawings prepared by KL&A, Inc. dated September 28,
2017.
Testing of Cast-in-Place Concrete
During the referenced period, EEC personnel observed and tested Portland cement concrete
delivered to the site for placement as indicated on the enclosed daily reports for the referenced
project, (please refer to Set No’s. 1 and 2). Field testing performed on the plastic concrete
included slump, air content, unit weight, and concrete temperature. The ambient air temperature
at the time of placement was also recorded. A minimum of five (4-inch diameter by 8 inches in
height) concrete test cylinders were cast from representative samples on each respective date for
laboratory curing and compressive strength testing. Results of the field and 7-day laboratory
compressive strength test results performed during the referenced period are included herein,
while the 28-day test results will be provided separately, as they become available.

As requested, during the period of January 8 through January 13, 2018, Earth Engineering
Consultants, LLC (EEC) personnel provided on-site construction observation and testing
services for the referenced project. The services provided by EEC personnel during the
referenced period were on a part-time/as-needed basis and included observations, field density,
and laboratory testing of footing overexcavation fill material as described herein. The results of
the field and laboratory testing services completed by EEC personnel during the referenced
period are included with this report.
Laboratory and Field Density Testing Services
During the referenced period, EEC personnel obtained one (1) representative sample of the on-
site overburden subgrade materials, (EEC Sample ID “A”). The composite sample was returned
to our office for further laboratory testing, evaluation and classification. Laboratory testing
performed on the sample included washed sieve analysis (ASTM C117 and C136), Atterberg
limits (ASTM D4398) and standard Proctor (ASTM Specification D698) tests. Results of the
laboratory testing completed by EEC personnel during the referenced period are shown on the
attached summary sheets. The results of the recently completed Proctor test were used to
evaluate the relative compaction of placed materials in the field.
During the referenced period, EEC personnel performed observations and field density tests on
foundation overexcavation fill materials that had been placed/prepared and compacted prior to
and/or during our site visits. The field density tests were completed at random locations and
