Dear Mr. Isgrig:
In accordance with your request, representatives of Olsson Associates have provided
Special Inspections and Construction Materials Testing services at the above referenced
site. The items observed include foundation bearing materials, structural fill placement,
reinforced concrete, post-tension tendon placement and elongation measurement,
structural steel observation, and placement of the ATS shear wall hold downs. Our
services were provided on a part to full-time basis between October 21, 2016 and
December 27, 2017 as scheduled by the general contractor.
Summary of Activities
Foundation Bearing Materials
The bearing materials in the bases of the grade-beam footing excavations in the following
locations were observed during this reporting period:
November 3, 2016 - Bearing evaluation along Lines A-C, 1-5
November 4, 2016 - Hand auger testing along Line B, 1-5
The bearing materials were evaluated with respect to the 1,500-psf design bearing
pressure. Hand auger testing was performed to sample material beneath the bearing
materials to delineate areas of deleterious debris. Encountered deleterious material
beneath the foundation systems and “ribs” of the post-tensioned floor slab between Lines
A-C, 1-5 was removed.

Earthwork Observation
Samples of on-site and imported materials were obtained for laboratory moisture-density
(standard Proctor), P-200, and Atterberg Limits testing. Field moisture-density tests were
performed on the structural fill placed in the following locations during this reporting
period:
October 21, 2016 - Utility trench backfill along Line B, 2-3
November 2, 2016 - Structural fill between Lines A-C, 1-5
Results of the field density tests are summarized in the attached reports.
Concrete Observation
Placement of the reinforcing steel and concrete in the following locations was observed
during this reporting period:
November 7, 2016 - Foundations and grade beams between Lines A-C, 1-5
November 8, 2016 - Foundations and grade beams between Lines A-C, 1-5
November 16, 2016 - Post-tensioned floor slab between Lines A-C, 1-5
November 19, 2016 - Post-tensioned floor slab between Lines A-C, 1-5
Placement of the reinforcing steel in the above referenced locations was observed on a
part-time basis. Field tests were performed and test specimens were cast with concrete
placed in these locations. Olsson’s field representatives observed the placement of
concrete on a full-time basis. Results of the compressive strength tests can be found in
the attached reports.
Post-Tensioned Concrete Observation
Olsson observed the marking and stressing of the post-tensioned tendons within the floor
slab of Building 8 on a full-time basis during this reporting period:
November 16, 2016 - Tendon layout in the floor slab between Lines A-C, 1-5
December 1, 2016 - 100% stressing of post-tension tendons Lines A-C, 1-5
December 22, 2016 - 100% stressing of post-tension tendons Lines A-C, 1-5
Olsson prepared post-tensioned tendon layouts to number and reference each individual
tendon. Final elongations were measured and reported to the post-tension slab designer
for approval. The measured elongations can be found in the attached reports.
Shear Wall Hold Down Observation
On November 15, 2017 and December 27, 2017, the placement of the ATS hold down
system hardware was observed including plate to anchor bolt welds, rod sizes and nut
connections.
Status of Compliance
This report represents Olsson's final inspection report for Building 8 at the Foothills Mall
Multifamily Apartment Complex in Fort Collins, Colorado. The special inspection items
observed and tested by our field personnel include foundation bearing materials,
structural fill placement, reinforced concrete, post-tensioned tendon placement, and
elongation measurements and placement of the ATS shear wall hold-down system. To
the best of our belief and knowledge, the items observed and/or tested by our field
personnel and explicitly reported in the attached test and inspection reports were in
general compliance with the project specifications or recommendations of the design
engineer.

Olsson Associates arrived on-site to observe post-tensioning of tendons within post-tension slab along the referenced location.
Upon arrival Olsson observed marking of tendons for measurement datum using a 6" wood block. Olsson observed the use of an
hydraulic pressure jack to pull tendons to 100% of design elongation. Olsson notes that tendons PT8-39, PT8-40, PT8GB-15 and
PT8GB-16 were not pulled due to excessive honeycombing and recommended that voids be patched prior to stressings. Olsson
will provide a completed post-tension observation report to the client at a later date once all tendons have been stressed. On-site
concrete foreman for GBC Concrete and on-site superintendent for Weitz were notified of these observations.

Olsson Associates arrived on-site to observe post-tensioning of previously in-stressed tendons within post-tension slab along the
referenced location. Upon arrival Olsson observed that honeycombing around previously un-stressed tendons had been patched.
Olsson observed the use of an hydraulic pressure jack to pull tendons to 100% of design elongation. Tendons PT8-39, PT8-40,
PT8GB-15 and PT8GB-16 were Pulled. Olsson will provide a completed post-tension observation report to the client at a later.
On-site concrete foreman for GBC Concrete and on-site superintendent for Weitz were notified of these observations.

Olsson Associates arrived onsite to perform observations of the ATS shear wall hold downs systems. Olsson
met with Trevor the subcontractor and walked all three buildings to verify that hardware was installed as per
plan. Upon observations of building 8 there were several sections that were not completed, Olsson informed the
contractor and Weitz that these sections should be completed and then rescheduled for reinspections.

Building 8 had hardware missing hardware for the (3) T1.4 rods, two on Grid C, 3 and one on Grid B.5, 5. Olsson
also was unable to view T3.7 rods at Grid C.2, 2.2 and C.2, 3.8 as these were covered by sheeting and will need
to be exposed for visual observations. Olsson should be notified of when these have been fixed for reinspection.
