HomeMy WebLinkAbout4803 Boardwalk Dr - Special Inspections/Engineering - 02/17/2017February 17, 2017 Aspen Summit, LLC 1223 Red Oak Court Fort Collins, Colorado 80525 Attn: Mr. Jesus Martin-Roman (martinj(a1forrcollin ·peri donti ·t.com) EARTH ENGINEERING CONSULTANTS, LLC Re: Construction Observation and Materials Testing Report - Final Summary Report Angie Lee Periodontics - 4803 Boardwalk Drive Fort Collins, Colorado EEC Project No. 1164091 Mr. Martin-Roman: As you are aware, during vanous construction phases for the recently completed Angie Lee Periodontics project located at 4803 Boardwalk Drive in Fort Collins, Colorado, Earth Engineering Consultants, LLC (EEC) personnel, provided construction observation and testing services of requested items on a part-time/periodic, as-needed/on-call basis as scheduled by the project's general contractor, Elder Construction as per the project specifications. Bi-monthly construction materials testing (CMT)/quality control (QC) reports concerning the services provided by EEC personnel were submitted to Aspen Summit, LLC (owner) and to Elder Construction, (the general contractor and owner's representative) during the appropriate time period(s) as the test results became available. We have recently been asked by Mr. Justin Robinson, project superintendent with Elder Construction, to provide a summary letter addressing the services performed by EEC personnel regarding but not limited to the foundation system, the interior floor slab, any structural related elements, and structural steel erection components/non-destructive testing/NOT services for the project. During the foundation construction phase for the referenced project, EEC personnel provided site observations and testing services of the foundation/footing system, including the over-excavation and replacement concept to allow for conventional spread footings as a swell mitigation approach for the new building footprint addition. The site observations and testing services during the foundation and interior floor slab construction phases were performed and completed, as recommended in the project-specific subsurface exploration report, (please refer to EEC's "Subsurface Exploration Report" dated November 2, 2015, EEC Project No. 1152104). 4396 GREENFIELD DRIVE WINDSOR, COLORADO 80550 (970) 545-3908 FAX (970) 663-0282 Angie Lee Periodontics - 4803 Boardwalk Drive Fort Collins, Colorado EEC Project No. 1164091 February 17, 20 l 7 Page 2 Earth Engineering Consultants, LLC EEC personnel provided site observations and field density testing during the over-excavation and replacement approach for the building footprint backfill zone material, which was placed and compacted as engineered fill material to enhance the bearing strata and underslab subgrades as per the project specifications. EEC personnel provided observation of the foundation system, (i.e., the conventional-type spread footings), and the interior floor slab reinforcement requirements. EEC personnel also provided testing of cast-in-place concrete for the foundation system and the interior floor slab. EEC personnel provided reinforcement observation of the foundation system(s) including the conventional spread footings placed on approved engineered/controlled fill material, cast-in-place foundation walls, and interior floor slab. EEC personnel provided testing of all cast-in-place concrete for the foundation system, interior floor slab and the exterior concrete flatwork areas. Based on our review of the outlined testing, as directed by the general contractor and completed by EEC personnel on an as-needed basis, the final field density test results within the building footprint, and associated exterior flatwork, along with site utilities, met the project compaction requirements for fill materials placed and compacted to redevelop the foundation bearing strata, interior floor slab elevations, backfill placed adjacent to foundation walls and within on-site utility trenches. Laboratory compressive strength tests completed on the cast-in-place concrete for foundation elements and floor slabs, conformed to the project specified minimum 28-day compressive strengths requirements. Observations of reinforcement within each of the foundation systems for the project, (i.e., the cast- in-place conventional spread footings, cast-in-place foundation walls/stem walls) were completed by EEC personnel and reports were submitted to your attention during the appropriate periods as the results became available. Based on our review of the outlined observations, we believe the reinforcement within the foundation system and structural related elements were placed and constructed in general compliance with the project plans and specifications and associated RFI's specific to the project. Observations for the structural steel erection, welding and bolt connections were completed by EEC's in-house ICC Bolt and Weld *Inspector, and reports were submitted to your attention during the appropriate period as the results became available. LLC the structural steel, weld, and bolted connections were in general compliance with the project plans and specifications. Additional copies of our six (6) periodic/bi-monthly construction materials testing (CMT)/quality control (QC) test reports dated October 15, 2016 - Submittal No. 1, November 8, 2016 - Submittal No. 2, November 18, 2016 - Submittal No. 3, December l, 2016 - Submittal No. 4, December 15, 2016- Submittal No. 5, and January 4, 2017 - Submittal No. 6 are available upon request.
