CO 80522 970.416.2740 Phone 970 224,6134 Fax PERMIT # !l Z,0QA10 I as owner(s) of record of the property located at '�,'a (0 E Y+1 0^^ N Fort Collins, Colorado, hereby declare and attest to the following: Fil-471WAAWIf. i ZZAi ❑ This is a residential remodel project, which is not my primary residence, for which the total construction value of all work including electric, plumbing and mechanical trades does not exceed $2,000 for any one such building project and is considered "Minor Alterations and Repairs" as noted on page 2, and does not involve structural work or alterations to any fire -resistive assemblies. ❑ I am acting on my behalf for the purpose of obtaining a building permit for minor alterations to the building referenced above. I am/we are personally performing all of the work as allowed by City of Fort Collins code, other than the work required by licensed trades as referenced above, and as approved on the permit documents with the same permit number as noted above. I understand that I will be asked to provide the City of Fort Collins a copy of bids from a# subcontractors that will be involved In any alterations to this project ❑ I/we understand that all work related to electrical, plumbing, HVAC, gas piping and fire extinguishing assembly alterations must be performed by a City of Fort Collins licensed subcontractor and that this work cannot commence until the City of Fort Collins Building Services department is notified of all the licensed subcontractors and a building permit obtained. No structural alterations or alterations to fire rated assemblies are allowed without a licensed General Contractor. COMMERCIAL 8UILDING OWNER $i� This is a commercial project for which the total construction value of all work including electric, plumbing and mechanical trades does not exceed $2,000 for any one such building project and is considered "Minor Alterations and Repairs" per City Municipal Code Section 15-155. - Exemptions (noted on page 2) and does not involve structural work or alterations to any fire -resistive assemblies. LK I am acting on my behalf for the purpose of obtaining a building permit for minor alterations to the building referenced above. I am personally supervising the licensed subcontractors and all the work as allowed by City of Fort Collins Code and as approved on this permit and approved permit documents. I understand that I cannot allow the tenant to do work requiring a City licensed contractor, they cannot act as the General Contractor, or perform other work not requiring a license (except as allowed under "Minor Alterations and Repairs") for which this permit is issued.
