BOX 200 CONSULTANTS, LLC 650 Innovation Circle Windsor, Colorado 80550 Attn: Ms. Amanda Martinez(amanda.martineznmccauIeyconstructors.coin) Re: Construction Observation and Testing — Final Summary Letter Lemay Self Storage —Building "D2" Permit No. B1704323 1057 Buckingham Street Fort Collins, Colorado EEC Project No. I t74099 Ms. Martinez: As you are aware, during various construction phases for Building "132", recently completed within the Lemay Self -Storage Facility, located at the southwest corner of Lemay Avenue and Buckingham Street in Fort Collins, Colorado, Earth Engineering Consultants, LLC (EEC) personnel, provided observation and testing services of requested items on a part-time, as- needed/on-call basis. Reports concerning the services provided by EEC personnel were submitted to McCauley Constructors, Inc. (the project's General Contractor) and to other project design team members' attention during the appropriate time periods as the test results became available. We have recently been asked by Mr. Lauren McCauley, Project Superintendent with McCauley Constructors, Inc., to provide a summary letter addressing the services performed by EEC's personnel during the construction phase from August 2017 through June 2018, regarding but not limited to the following items: foundation observations, reinforcement observations of the foundation system, (i.e., spread footings/turn down slab and walls), non-destructive testing (NDT) observations and services associated with the structural steel erection components and light gauge framing elements for the project, earthwork related elements associated with the proposed development including building pad and foundation bearing strata preparation, utility trench backfill and site improvements, along with testing of cast -in -place concrete During the foundation construction phase for the referenced project, EEC personnel provided site observations and testing services for foundation/footing system and interior floor slab for Building "132". The site observations and testing services during the foundation and interior 4396 GREENFIELD DRIVE WINDSOR, COLORADO 80550 (970) 545-3908 (FAX) 663-0282 www.earth-engineering.com Earth Engineering Consultants, LLC Lemay Self Storage — Final Summary Letter for Building "D2" EEC Project No. 1174099 June 19, 2018 Page 2 floor slab construction phases were performed and completed, as recommended in the project - specific subsurface exploration report, (please refer to EEC's "Subsurface Exploration Report" dated April 26, 2016, EEC Project No. 1 162031). EEC personnel provided site observations/foundation excavation observation of the bearing strata, and field density testing of engineered fill material placed and compacted to enhance the bearing strata and underslab subgrades as per the project specifications. EEC personnel provided reinforcement observation of the foundation system including the conventional spread footings/turn-down slab placed on approved engineered/controlled fill material, cast -in -place concrete elements and interior floor slab. EEC personnel also provided testing of all cast -in - place concrete for the foundation system, interior floor slab(s), along with the exterior concrete flatwork areas. Based on our review of the outlined testing, as directed by the general contractor and completed by EEC personnel on an as -needed basis, the final field density test results within the building footprint's foundation and floor slab areas, and sitework/utilities, met the project compaction requirements for fill materials placed and compacted to redevelop foundation bearing strata, interior floor slab elevations, and backfill placed adjacent to foundation walls. Laboratory compressive strength tests completed on the cast -in -place concrete for the foundation related elements and floor slabs (both interior and exterior) conformed to the project specified minimum 28-day compressive strengths requirements. Observations of reinforcement within the foundation system, and the interior and exterior flatwork areas for the project, were completed by EEC personnel and reports were submitted to your attention during the appropriate periods as the results became available. Based on our review of the outlined observations, we believe the reinforcement within the foundation system, and interior and exterior slabs were placed and constructed in general compliance with the project plans and specifications. Observations for the structural steel erection, welding and bolt connections and light gauge framing were completed by EEC's in-house ICC Bolt and Weld *Inspector, and reports were submitted to your attention during the appropriate period as the results became available. The work that was visually observed in the areas of the referenced gridlines as previously reported, appeared to be in general accordance with the project plans and details dated December 6, 2016.
