Dear Mr. Isgrig:
In accordance with your request, representatives of Olsson Associates have provided
Special Inspections and Construction Materials Testing services for Building 12 at the
above referenced site. The items observed include foundation bearing materials,
structural fill placement, reinforced concrete, post -tension tendon placement and
elongation measurement and placement of the ATS shear wall hold down system. Our
services were provided on a part to full-time basis between July 7, 2016 and March 16,
2017, as scheduled by the general contractor.
Summary of Activities
Foundation Bearing Materials
The bearing materials in the bases of the grade beam footing excavations in the following
locations were observed during this reporting period:
August 02, 2016 - Hand auger testing between Lines 1-5, A-C
August 31, 2016 - Line A, 1-5; Line 1, A-C
September 01, 2016 - Line C, 1-5; Line 5, A-C
The bearing materials were evaluated with respect to the 1,500-psf design bearing
pressure. Hand auger testing was performed to sample material beneath the bearing
materials to delineate areas of deleterious debris. Deleterious material was encountered
beneath the foundation systems and "ribs" of the post -tensioned floor slab between lines
A-C, 1-5. The contractor elected to over -excavate and backfill the material in these
locations.

Earthwork Observation
Samples of on -site and imported materials were obtained for laboratory moisture -density
(standard Proctor), P-200, and Atterberg Limits testing. Field moisture -density tests were
performed on the structural fill placed in the following locations during this reporting
period:
July 13, 2016 - 2 to 3-feet below Finished Floor Elevation along Line A, 1-5
July 14, 2016 - 1.5-feet below Finished Floor Elevation between Lines B-C,
1-5
July 15, 2016 - 1-foot below Finished Floor Elevation between Lines B-C, 1-5
July 18, 2016 - 0.5 to 1-feet below Finished Floor Elevation between
Lines A-C,1-5
August 26, 2016 - 0.5 to 3-feet below Finished Floor Elevation for plumbing
between Lines B-C, 1-5
August 29, 2016 - 0.5-feet below Finished Floor Elevation for plumbing along
Line B, 1-5
Results of the field density tests are summarized in the attached reports.
Reinforced Concrete
Placement of the reinforcing steel and concrete in the following locations was observed
during this reporting period:
September 08, 2016 - Shallow foundations and grade beams between Lines A-C,
1-5
September 29, 2016 - Post -tensioned floor slab between Lines A-C, 1-5
October 03, 2016 - Post -tensioned floor slab between Lines A-C, 1-5
Placement of the reinforcing steel in the above referenced locations was observed on a
part-time basis. Field tests were performed and test specimens were cast with concrete
placed in these locations. Olsson's field representatives observed the placement of
concrete on a full-time basis. Results of the compressive strength tests can be found in
the attached reports.
Post -Tensioned Concrete
Olsson observed the marking and stressing of the post -tensioned tendons within the floor
slab of Building 12 on a full-time basis:
September 29, 2016 -Tendon layout in the floor slab between Lines A-C, 1-5
October 04, 2016 - 50% stressing of post -tension tendons
October 11, 2016 - 100% stressing of post -tension tendons
Olsson prepared post -tensioned tendon layouts to number and reference each individual
tendon. Intermediate and final elongations were measured and reported to the post -
tension slab designer for approval. The measured elongations can be found in the
attached reports.
ATS Shear Wall Hold Downs
Placement of the ATS hold down system hardware was observed during this reporting
period. On March 16, 2017, the placement of ATS hold down system hardware including
plate to anchor bolt welds, rod sizes and nut connections, stud nailing patterns and plate
to floor heights were observed
Status of Compliance
This report represents Olsson's final inspection report for Building 12 at the Foothills Mall
Multifamily Apartment Complex in Fort Collins, Colorado. The special inspection items
observed and tested by our field personnel include foundation bearing materials,
structural fill placement, reinforced concrete, post -tensioned tendon placement and
elongation measurements and placement of the ATS shear wall hold-down system. To
the best of our belief and knowledge, the items observed and/or tested by our field
personnel and explicitly reported in the attached test and inspection reports were in
general compliance with the project specifications or recommendations of the design
engineer.

Monroe & Newell Engineers, Inc. observed Lot 4 Building 12, 3301 Stanford Rd. Permit
#B 1601314 during construction.
To the best of our knowledge and belief, based on our visual observations, and the
information provided to us, the foundation and wood framing appears to be constructed in
compliance with the intent of the construction documents prepared by our office and
approved by the Building Department.
