CSE Consultants visited the above -referenced project on April 10, 2017 as requested The purpose of this visit was
To observe the following five items
1 A 91/'2" deep second floor ljolst spanning east -west located approximately 14V2 feet south of the front, door has the
north face of the I joist top chord cut out for electrical wiring at a location of approximately at 3'-6" to 4'-0" east of
the centerlme of the W 1 Ox?2 steel second floor beans
Refer to "Item I Reinforcing" per the attached sheet SX I for repair plans
_ 2 A91/2" deep second floot 1-joist spanning east -west located approxuliatcly 13 feet south of the front door has the
,-syeb cut out for a flexible mechanical duct at a distance of approximately T-31," west of the eenterline_of the N7V1 Ox22
steel second floor beam The web cut for the mechanical duct is closer to the end.of the joist than permitted u) the
manufacturer user guide -

3 A double 11 7/8" deep second floor I ;joist spanning north -south above the garage located appioannately 4 feet east
of the garage exterior west wall has the webs cut out for a flexible mechanical duct at a distance of approximately
4'-0" from the north end of the joist The web out for the mechanical duct is closer to the end of the joist than
peumtted in the manufacturer user guide This double joist was specified to be a (2) 1 3/4 x 11 7,18 LVL beans on the
plans
- The joists used (PW147) exceed the properties of the floor Joists specified The double joists used In lieu of the
double LVL specified and tlus'web hole ns deemed acceptable„ _
4 The 11 7/8" deep second floor 1-joists spanning north -south above the garage that are parallel with and adjacent
to the (3) 1 3/4 x 11 7/8 LVL flush joist in the second floor system are not located 1'-0" east and 1'-0" west of the
Lb L flush joist as indicated on the plans The west joist centerline is located 1'-7" west of the LVL center line and
the east joist centerline is located 12 3/4" east of the LVL centerline.
- This item is deemed acceptable
5 The basement level steel columns are incorrect (C1 and C2 columns installed as incorrectly switched)
- install the correct C 1 and C2 columns at the locations indicated on the plans
