Mr. Frakes,
Per your request, our office has performed the Construction Phase Consulting Services for the above mentioned
project including the following:
• Review of concrete mix designs supplied by Innovative Concrete Solutions.
• Review of stressing equipment calibration logs supplied by Innovative Concrete Solutions.
• Review of observation logs for post-tension concrete stressing supplied by Terracon.
• Request For Information (RFI) submittal review and response.
Our previous field observation report dated June 5, 2014, noted deficiencies within the post-tension slab-on-grade at
the above mentioned project. It is our understanding, as relayed by Innovative Concrete Solutions, that all of the
deficiencies have been corrected and that all stressing operations have been completed.
Based on our review of the information noted above, Anchor Engineering, Inc. feels that the post-tension concrete
slab-on-grade foundation for the above mentioned project has been completed in general conformance with the
structural foundation drawings prepared by our office.
