Per your request, we have provided the following inspections for the single family residence being
constructed at the above referenced address:
1.On December 22, 2014 we inspected the continuous footing form sizes, footing pad sizes, void form
material and steel reinforcement, prior to the placement of concrete.
2.On January 6, 2015 we inspected the steel reinforcement within the foundation wall, prior to the placement
of concrete.
3.On January 21 and January 30, 2015 we inspected the exterior perimeter drain systems and foundation
wall damp-proofimg, prior to the placement of backfill material.
These inspections consisted of visual observations and measurements to confirm that the footing form
widths and depths, footing pad sizes, void form material, footing steel reinforcement, foundation wall steel
reinforcement, covered patio and portico footing pads and piers, tie -down straps, concrete encased electrode,
exterior perimeter drain system and foundation wall damp -proofing were in place and within general
conformance with the approved plans completed by Straigbt Line Design, Inc., job #14-1987, dated
September 4, 2014 or per our verbal recommendations. Upon completion of each inspection, we have
determined that all requirements pertaining to the inspections have met or exceeded our recommendations.
