On April 14, 2015, Earth Engineering Company, Inc. (EEC) personnel provided construction
observation services you requested for the referenced project. Those services included observation of
the grade beam reinforcing for the detached garage prior to the placement of cast -in -place concrete. Two
#5 and/or 4 #4 horizontal rebar were placed in the grade beams at the top and bottom of the grade beam
walls. Lap lengths were checked and appeared to meet the minimum required lengths.

Dear Mr. Brex:
At your request, representatives of Quality Engineering, LLC were present on March 6, 2015, and March
18, 2015 to observe post -tensioned slab formwork, tendons, reinforcing placement, and stressing. These
observations and inspections were made to confirm that post -tensioned slab formwork and reinforcing
placement were completed in substantial compliance with the direction, details, and specifications shown
on the structural plans by Quality Engineering, dated March 1, 2015. Post -tensioned slab formwork,
tendons, reinforcing placement and stressing were completed in substantial compliance with the direction,
details and specifications noted on the referenced structural plans.
