As requested, the following portions of the foundation system were observed on the date(s)
indicated below. It is our opinion that these portions of the foundation systern were found to be in
substantial conformance with the foundation plan and requirernents of the soil report for the subject
site. Observations were conducted on a periodic, on -call basis.

As requested, density tests were taken on a periodic basis on the fill being placed at the
site of the subject project. Submitted are Daily Report Number(s) 1 and 2 covering
observations of the fill through the period ending February 20, 2014.
The density test results met the specifications provided by our office.
These tests should be considered as periodic observations. The placement of the fill was
not continually observed by our firm. The test depths and locations are approximate and
are based upon information provided by the Contractor. The lateral and vertical extents
of any subexcavation were not measured or verified by this firm. The Contractor has
been informed of the field density test results.

PROGRESS REPORT: As requested by the Contractor, we visited the site several time(s) today. The following
information summarizes our observations at the time of our site visit(s). The Contractor, Blanding, began backfill of
the excavated area. The Contractor placed crushed gravel in one foot lifts to approximately one to two feet above the
water table. A front end loader wheel rolled each lift in. No fill was placed in the excavation today.
The Contractor was informed of our observations, and recommendations. Observation services were conducted on an
on -call basis.
