RE: Streamside Project
To Whom It May Concern:
This letter serves as an agreement between Gallegos Sanitation and Cross Creek Builders
for the Construction Waste Management Plan (CWMP) of the Streamside Project. The
intention of the CWMP is to clarify the services that Gallegos Sanitation offers construction
clients. If your company plans to recycle project materials this form must be completed prior to
delivery of equipment.
Please review the following sections of this agreement. Please initial grey -highlighted areas
stating you have read our policies. Please return this form to our office electronically or via fax
(970-484-0662).
Section 1: SEPARATION & CONTAMINATION
1 Recycle Enclosure: Individual material stream dumpsters will be delivered to the central
construction site location as ordered by Cross Creek Builders.
=Separation: Cross Creek Builders is responsible to label dumpsters, direct sub-
contractors to separate waste, and put the correct material into the designated dumpsters.
( ------'.Contamination: If GSI driver sees contamination such as bottles, cans, plastic, ect- GSi
will not haul the load to be recycled, and/or will haul the load as trash. A contamination/trip
charge of up to $300 per haul in addition to the original haul fee will be assessed.
Section 2: ANTICIPATED PROJECT WASTE & GUIDELINES
Please initial the'grey-box for each material you anticipate to have GSI haul from the project
F --11 Metals- to be recycled at Aragons iron and Metal- 516 US HWY No. 287, 80524
Accepted items: All metals mixed together -Steel, copper, aluminum, aluminum cans,
brass, aluminum; metal strapping, aerosol cans, clean rebar, nails
Not Accepted: No Trash, plastic, paper
Concrete —To be recycled at City FC Streets- 1380 Hoffman Mill Rd. 80524
Accepted Items: Clean Concrete, Grout, Ceramic Tile, Porcelain
Not Accepted: No METAL of any kind, NO strapping or rebar, No Plastics, NO TRASH

Cardboard— To be recycled at Larimer County Recycle Center- 5887 South Taft Hill
Road, 80525
Accepted Items: Clean Cardboard Boxes only
Not Accepted: No wax board, paint contaminated or oil soaked cardboard, no food
stained products, no Styrofoam, no plastic bags or strapping.
Wood- To be recycled at Hagamens Earth Cycle- 3501 1 /2 E Prospect 80525
Accepted: Clean Wood/Lumbar ONLY — Nails OK; OSB OK- Trim, Pallets OK
Not Accepted: No Paint or treated wood, No labels/stickers, or No plastics shrink wrap,
No laminate, no packaging slips
- -- Trash- To be taken to Weld County Landfill- Hwy 14, Ault CO
Accepted: Gypsum, Painted/treated wood, plastic strapping, plastic shrink wrap, plastic
bags, PVC Piping and fencing, Styrofoam, dry paint cans
Not Accepted: Clean Cardboard, Clean Lumbar, Metal, Hazardous Waste, Tires
