On September 18, 2013, Earth Engineering Company, Inc. (EEC) personnel provided
construction observation services you requested for the referenced project. Those services
included observation of the grade beam reinforcing prior to the placement of cast -in -place
concrete. Two #4 and single #5 horizontal rebar were placed in the grade beams at the top
and bottom of the grade beam walls, respectively at various locations. Vertical #4 bars were
observed at approximately 2' on -center throughout the length of the grade beams. Lap
lengths were checked and appeared to meet the minimum required lengths.
At the time of our observations, it appeared that the wall reinforcing steel had been placed in
general accordance with the construction plans prepared by Barton Supply dated July 31,
2013.

As requested, Earth Engineering Company, Inc. (EEC) personnel provided construction
observation and testing services to you for the referenced project. Those services were
provided on a part-time basis and included testing of cast -in -place concrete for the
referenced project.
On November 13, 2013, EEC personnel observed and tested Portland cement concrete
delivered to the site for placement at the post -tensioned slab -on -grade. Field testing
performed on the plastic concrete included slump, air content, and concrete temperature.
The ambient air temperature at the time of placement was also recorded. Concrete test
cylinders were cast from representative samples for laboratory curing and compressive
strength testing. Results of the field and laboratory tests performed during the referenced
date are provided with this report.
