Schaffer(d,anheuser-busch.com) Mr. Dennis Wallisch (Dennis.Wallisch(a,anheuser-busch.com) Re: Construction Observation and Testing — Final Compliance Report Anheuser Busch - South Entrance Sign Installation of Two (2) Friction Piers for Entrance Sign 2351 Busch Drive Fort Collins, Colorado EEC Project No. 1134066 Mr. Schaffer and Wallisch: As requested, Earth Engineering Consultants, LLC (EEC) personnel provided construction observation and testing services for the referenced south entrance roadway improvement project on the premises of the ABInBev (Anheuser-Busch) of Fort Collins Brewery property. The services performed were provided on an essentially full-time basis and included observations of the two (2) friction pier installed on February 24, 2014 for the overhead entrance sign and testing of cast -in -place concrete for the referenced project. The results of the site observations and testing services completed by EEC personnel during the referenced period are included with this report. On February 24, 2014, EEC personnel provided observation services for the two (2) 42-inch diameter friction piers for south entrance roadway overhead sign. Those services were essentially provided on a full-time basis and included observations of friction pier diameter (42- inches), depth to installation, (approximately 15 to 15-1/2-feet below site grades), friction pier length,(total pier length with portion raised above site grades equaled 16-feet), presence of groundwater, (observed at an approximately depth of 11-feet below site grades), and displacement/dewatering methods, (driller slurry methodology and use of a concrete pump truck to displaced water at surface), reinforcing steel, and testing of cast -in -place concrete. 4396 GREENFIELD DRIVE WINDSOR, COLORADO 80550 (970) 545-3908 (FAX) 663-0282 www.earth-engineering.com �3S / 1).)5'ch PL Earth Engineering Consultants, LLC Anheuser-Busch— South Entrance Sign Final Compliance CMT/QC Report Fort Collins, Colorado EEC Project No. 1134066 May 12, 2014 Page 2 The reinforcement in general, consisted of 12 #1 0 vertical rebar with 11 #4 horizontal ties evenly spaced. The top portion consisted of two (2) rows of cross -tie reinforcement, in general accordance with detail B-B/9021 as presented on the structural drawings. The reinforcement and installation of the anchor rods were in general accordance with the. details 3/9021, A-A/9021 and B-B/9021 of the Revised Structural drawings prepared by CSD Project No. FCB-18-C-9021 Rev. 2 dated January 30/2014. During the friction pier installation phase, EEC performed observations and testing services of the cast -in -place concrete as indicated on the enclosed daily report, (Test No. 3). The concrete testing was performed in general accordance with American Concrete Institute (ACI) specifications. Field testing performed on the plastic concrete included slump, air content, unit weight, and concrete temperature. The ambient air temperature at the time of placement was also recorded. A minimum of 5 (4" x 8") concrete test cylinders were cast from representative samples for laboratory curing and compressive strength testing. Results of the field and laboratory compressive strength tests results performed are included herein. We appreciate the opportunity to be of service to you on this project.
