Century Environmental Hygiene PLM Laboratory (CEH PLM Lab) is an accredited laboratory
by the National Voluntary Laboratory Accreditation Program (NVLAP), Lab Code 500072-0, for
Polarized Light Microscopy (PLM) analysis. This laboratory is currently proficient with the
NVLAP Proficiency Testing Program.
CEH 1135 is the laboratory number assigned to this report. CEH PLM Lab has analyzed the bulk
samples you submitted by polarized light microscopy (PLM). The analysis has been completed
in general accordance with the appropriate EPA recommended methodology. Stereoscope
microscopy and polarized light microscopy (PLM) coupled with dispersion staining is the
technique used for sample identification. The percentage of each component is visually estimated
by volume. These samples were analyzed as submitted by the client and may not be
representative of the layer of material in question.
This report is considered highly confidential and the sole property of the client. CEH PLM Lab
will not discuss any part of this report with personnel, other than the client. Unless notified,
samples will be disposed of after six months from date received. The results in this report only
apply to the samples analyzed. This report must not be used to claim endorsement of products or
analytical results by NVLAP or any agency of the U.S. Government. This report shall not be
reproduced except in full, without permission from CEH PLM Lab. If you have any questions

The samples were submitted to Century Environmental Hygiene, LLC analytical
laboratory in Fort Collins, Colorado who is accredited by the National Voluntary
Laboratory Accreditation Program (NVLAP). The samples were analyzed for
asbestos content concentrations utilizing Polarized Light Microscopy°(PLM).
ASBESTOS CONCENTRATION GUIDELINES
The State of Colorado, Department of Public Health and Environment (CDPHE)
considers a regulated asbestos -containing material as having at least 1% asbestos
content. Regulated asbestos -containing materials consist of the following: (a) friable
asbestos -containing material, (b) Category I non friable ACM that has become
friable, (c) Category 1 non friable ACM that will be or has been subjected to sanding,
grinding, cutting, or abrading or (d) Category 11 non friable ACM that has a high
probability of becoming or has become crumbled, pulverized, or reduced to powder
by the forces expected to act on the material in the course of demolition or
renovation operations. Friable materials containing trace (less than 1%)�
concentrations of asbestos content must be further analyzed by point -count analysis
to provide a quantitative result.
The percent of asbestos contained in drywall and its associated materials, including.
mud and tape, may be determined based on a representative composite sampling
program. Drywall finishing mud and joint compound frequently has been found to

contain a significant amount of asbestos (i.e. greater than 1%), while the substrate it
is applied over may or may not contain any asbestos. A demolition/renovation
project may impact the mud alone, such as could be the case during a sanding
operation or the demolition/renovation project may impact the composite material
(the mud and the substrate drywall it is applied over), such as would be the case in
the demolition of the drywall. In the first case, if the mud contains greater than 1%
asbestos, the project must be handled as an asbestos abatement project, subject to
all the applicable requirements of the Air Pollution Control Division. In the second
case, the project is considered to be an asbestos abatement project if the percent of
asbestos in the combined mud/drywall composite material is greater than 1%. If, on
the other hand, the mud contains 1% or less of asbestos, the material is not
considered to be an asbestos -containing material for purposes of the Air regulations
and is not subject to the asbestos abatement requirements. Similarly, if the
composite analysis indicates an asbestos content of 1% or less, even though the mud
itself may be greater than 1%, the material is not considered to be an asbestos -
containing material and does not fall under the asbestos abatement requirements of
the Air regulations. In both of these cases, the removed materials may be disposed
of as construction and demolition debris. Adequate documentation must be kept to
demonstrate the determination regarding the concentration of asbestos in the
materials being disposed.
REMODELING AND DEMOLITION GUIDELINES
Removal or management of regulated asbestos -containing materials is required
only if the material can be damaged or disturbed during remodeling, demolition, or
burning activities. If the amount of asbestos -containing material to be disturbed
exceeds the following trigger levels, then an asbestos permit will be required and an
abatement contractor must remove the material.
■ With regard to single-family residential dwellings, the trigger levels are 50
linear feet on pipes, 32 square feet on other surfaces, or the volume
equivalent of a 55-gallon drum.
■ With regard to all areas other than single-family residential dwellings, the
trigger levels are 260 linear feet on pipes, 160 square feet on other surfaces,
or the volume equivalent of a 55-gallon drum.
A certified removal contractor (General Abatement Contractor) must remove
asbestos -containing materials that are regulated or may become regulated before
they are disturbed by renovation or demolition activities. A written notification to
CDPHE, payment of a notification fee, and ten (10) working day waiting period is
required before the removal (abatement) of regulated asbestos containing
materials.

According to the CDPHE, certain types of non -friable asbestos -containing materials
(tar/asphalt-impregnated roofing, floor tiles, etc.) may not be required to be
removed prior to demolition. However, according to the CDPHE Hazardous
Materials and Waste Management Division's Asbestos -Contaminated Soil Guidance
Document, asbestos -containing materials are prohibited to impact soils. Any
asbestos -containing materials left in -place during demolition activities must be
removed with no residual debris.
CDPHE requires a demolition permit for all demolition activities (including
burning), even if a building does not contain asbestos. CDPHE also requires a
demolition permit for remodeling activities that involve the destruction or removal
of a load -supporting structural member. CDPHE requires ten (10) business days to
process the form and grant permission to begin demolition activities. A copy of the
CDPHE Demolition Permit Application is included in the enclosures section•of.this
letter report.

RECOMMENDATIONS
Based on the findings of this survey, National Inspection Services® recommends -the
following:
• National Inspection Services® identified asbestos -containing red tile material
located on the east -central storage room floor (north hallway section) and
under the carpet of the showroom floor area. National Inspection Services®
also identified asbestos -containing orange sheet vinyl backing material

located on the restroom and storage closet floors and under the carpet of the
showroom floor area. Due to carpet coverage and product shelving
components, the quantities of the red tile material and orange sheet vinyl
backing material located under the carpet could not be determined. If the
asbestos -containing red tile material and orange sheet vinyl backing material
will be disturbed during future renovation activities, National Inspection
Services® recommends that the red tile material and orange sheet vinyl
backing materials be abated by a certified removal contractor (General
Abatement Contractor) prior to renovation activities. For a list of General

LIMITATIONS
This survey was designed to provide the client with observable materials (materials
which were readily accessible and visible) that were collected and analyzed for
asbestos content. This survey does not include materials that were covered or hidden
by walls, structures, carpeting, multiple layers, texturing, etc.. Asbestos -containing
materials, not specifically tested or identified in this survey, may exist in areas that
were inaccessible or would require demolition or deconstruction activities to facilitate
identification (i.e. behind covered walls and/or ceilings, inside areas of ductwork
and/or mechanical equipment components, etc.). If any additional suspect asbestos-
containing materials not specifically tested or identified in this survey are identified
during future demolition activities, testing of the newly identified materials will be
required. This survey was limited in nature and should not be used as a
comprehensive survey for building demolition purposes.
Performance of this asbestos survey is intended to reduce, but not eliminate,
uncertainty regarding the presence of asbestos -containing building materials. National
Inspection Services® neither expresses or implies a guarantee that all asbestos-
containing materials were identified during this survey. National Inspection Services®
used reasonable diligence consistent with industry standards to identify asbestos -
containing materials during this survey.
This report has been expressly prepared to deliver the results of a physical survey
based on the collection of, and the laboratory analysis of, suspect asbestos samples:
National Inspection Services® does not engage in the abatement of asbestos
containing materials, and as such will not dispense abatement methodology advice.
Revisits to any subject property post -abatement, fof the purpose of Vdhdabn�grthe,,
abatement work of other parties, or for the preparation. of. state, certification.
documents, will accrue additional fees. Recommendations made by National
Inspection Services® are derived from general interpretations of CDPHE guidelines
