As requested, the following portions of the foundation system were observed on the date(s)
indicated below. It is our opinion that these portions of the foundation system were found to be in
substantial conformance with the foundation plan, a fax prepared by Advanced Engineering
Concepts, Inc. dated December 3, 2010 (stamped and signed), and requirements of the soil report
for the subject site. Observations were conducted on a periodic, on -call basis.
