I am/we are acting on my/our behalf for the purpose of obtaining a building permit and personally constructing an alteration or addition to my/our house The house to be altered is on the above property and is my/our personal residence ❑ 1 am/we are acting on my/our behalf for the purpose of obtaining a budding permit and personally constructing a non structural alteration to my/our attached single family dwelling unit The house to be altered is my/our personal residence I am aware that I/we cannot do any structural electrical plumbing ore mechanical work and must hire contractors/subcontractors who are currently licensed and insures with the City of Fort Collins I am/we are personally g of r o ill be continuously supervising unpaid volunteers The work is directly rel d t�i o t11 fin Uhqjabove referenced home which is described more fully on Permit Applications-++ (obtained from Budding and Zoning office) I/we understand that any person(s) or agent(s) contracted to perform structural wood framing plumbing HVAC electrical work roofing or cast -in place structural concrete work MUST BE licensed contractors in accordance with the regulation of the City of Fort Collins /we understand that failure to comply with any of the above conditions may result in revocation of any permits associated with the above Permit Application number forfeiture of any fees that have been collected a Stop Word Order and a court summons Sign in the presence of Notary Public Owner Co owner The foregoing Affidavit was acknowledged before me on this ' / V-1-1 day of (month year) byq Witness my hand and official My commission expires I• �P" My COMMISStOMEXP00 J* 23 2011 Notary Pubic Planning, Development & Transportation Services City of Fort Collins Community Development & Neighborhood Services 281 North College Avenue P O Box 580 Fort Collins CO 80522 0580 970 416 2740 970 224 6134 fax fcgov com 81004257 Homeowner Issued Building Permits Responsibilities of the Homeowner City of Fort Collins Municipal Code Section 15-155(2) A property owner may obtain building permits for any work without obtaining a general contractor license In the eyes of the City, the owner becomes the contractor of record for the project and assumes construction responsibilities same as if that owner was a licensed general contractor The City has the expectation that the homeowner complies with and can fulfill the following responsibilities 1 The property is a detached single family dwelling and the owner s principal residence 2 Application and permit process a The owner shall personally submit the application and appropriate plans b The owner shall be the sole contact person addressing all questions and staff interaction during the plan review c Upon approval of the application, the owner shall be present to sign the permit 3 Construction a The owner shall perform all work associated with the permit or, b Shall provide continuous supervision of all unpaid volunteers 4 Inspections a The owner shall schedule all inspections and be available on -site during each inspection b Inspections will be performed between the hours of 9 00 AM and 3 00 PM c For scheduled inspections where the owner is not present, the inspection will not be performed d A re inspection fee of $50 00 could be assessed for any scheduled inspection where the owner is not present 5 The owner takes full responsibility for complying with all Codes and Standards 6 By sigmng the building permit, the owner agrees to the above responsibilities Michael W Gebo CBO� June 21, 2010
