At your request, we have reviewed the construction materials testing documentation prepared
by Terracon during construction phases of the referenced project. Our services on the project
have included performing laboratory testing of the soils for compliance with the project
specifications and the Geotechnical Engineering Report recommendations prepared by Empire
Laboratories, Inc. (dated October 10, 1991; Project No. 9047-91) and the Geotechnical
Engineering Report for the proposed elevator prepared by Terracon (dated February 20, 1997
Project No. 20975014). Terracon also performed in -place field density testing during placement
of the fill to check maximum density and moisture control. We also performed reinforcing steel
observations, structural steel/weld/bolt observations, and concrete testing. All testing and
observations were performed on an "as requested" basis.
Earthwork
As recommended in the soils investigation, on -site or approved imported soils were suitable for
placement during construction. Laboratory tests indicated fill material tested by Terracon met
the recommendations as outlined in the geotechnical report. The results of the field and
laboratory testing were forwarded for review. In addition, the on -site superintendent and
subcontractors were informed on a daily basis of the test results performed.
Terracon performed 21 in -place moisture-density/compaction tests of the subgrade for the curb
and gutter, sidewalk, and parking lot. The tests at the locations indicated met or ultimately
achieved (after retesting) compaction specification requirements with the exception of report
No. 20081017.0012(attached for reference).
Our test results represent only the materials at the location and time of testing. Terracon did
not direct nor control the fill placement and compaction at the site and does not assume
responsibility for the placement, compaction or control of the fill materials. Areas of fill which
are not representative of our testing may exist, particularly in common fill areas such as slopes
or landscaped areas outside the limits of the building pad.

Concrete
Terracon
The Portland cement concrete placed and tested on -site consisted of 5 tests for physical
property and compressive strength tests of the footing foundation(s), second floor slab, and
sidewalk. The reports indicate the specified minimum compressive strength requirements for
the project have been achieved. It is recommended that the project engineer of record review
the reports for acceptance as meeting the project criteria.
Reinforcina Steel
Terracon provided reinforcing steel placement observations in the east and west footing
foundation(s) concrete placements as requested. The observations were performed per the
provided plans and specifications for the project and have been formally reported to the client
and designated parties. It is recommended that the project structural engineer review the
observation reports for acceptance as meeting the project criteria.
Structural Steel/Weld/Bolts and Fire Proofing Observation
Intermountain Testing Company provided structural steel/weld/bolted connection and
fireproofing observations during site construction. Observations were provided to on -site
personnel at the time of the visit with a written report as documentation. The observation report
dated March 26, 2008 and associated RFI No. 17 are attached for reference. It is
recommended that the project structural engineer review the observation reports for
acceptance as meeting the project criteria.
Foundation
Terracon provided observation of the footing foundation(s) and open -hole excavations as
requested during construction. A summary of the observations and structural review response
items were provided in written reports upon completion. Per Terracon's geotechnical report and
observations performed, foundation(s) have been placed in general accordance with the plans
and specifications and were placed on the anticipated bearing material identified in our
geotechnical report.
Closure
This report has been prepared for the exclusive use of providing test data and observations to
document the reinforcing steel, structural steel/weld/bolt, and concrete testing operations
conducted for the proposed project. The opinions and conclusions presented have been
derived in accordance with generally accepted geotechnical and material engineering practice.
No other warranty expressed or implied is made. In the event that there are any changes in the
nature or design of the project, the conclusions contained in this report shall not be considered
valid unless this firm reviews such changes, and the conclusions of this report are modified or
verified in writing.
