Our office was asked by Steve Josephs of Craftsmen Builders to inspect both the footing and foundation of the two- story WOO' guest house addition being built at 1122 West Mountain Avenue. Our office provided engineering support and the stamped plans submitted to the building department. On December 6, 2002 I inspected the footings for the perimeter walls. The continuous rebar was in place with the verticals ready to install during concrete placement. Concrete was scheduled for Ipm but had been moved back to later in the afternoon. It is our opinion that the footers meet the intent of our design. On December 9, 2002 I inspected the wall forms. The horizontal and vertical rebar was in place and the forms appeared to be located appropriately. Due to supply problems, the contractor did not have 5/8" anchor bolts on site. Because of this, we allowed the use of/s" anchor bolts 3' c/c instead of the 5/8" anchor bolts 5' c/c. The concrete was scheduled for later in the afternoon. It is our opinion that this foundation meets the intent of our design. Please call if there are any questions. This inspection and letter are limited to the specifrc items listed and in no way implies a cerVkation of the stricture, its systems or site work. No liability is assumed for future loss of value, marketability or any other loss claims. File: Craftsmen 1122 W Mountain 12-12-2002 FFI Job Reference: 02-283
