See the following LIST Of: VIOLATION(S). Please complete all repairs within 45 days from the date of this letter. When repairs are done, sign and date the bottom of this Notice and mail or fax it to the Boiler Inspection Office. If you cannot complete repairs within 45 days, call the Program Manager at 303-318-8481 or 303-318-8506 to request an extension. Operating a non -compliant boiler or pressure vessel is prohibited by Colorado law. [See 9-4-108(2) CRS] If repairs to this vessel are: not completed timely, the vessel may be flagged for shutdown and a reinspection fee may be charged. LIST OF VIOLATION(S) IDENTIFIED: (*)The safety limit temperature control is not equipped With a manual reset (lockout). Install safety limit temperature control with lockout requiring manual reset as per ASME CSD-1,CW-410(C).
