I understand that all other applicable . provislons of the Fort Collins Energy Code, including
following the. Fb1t Collins Insulation Guidelines, submitting an. insulation certification form and a
HVAC'eer t fication form, -obtaining a completed "Insulation Inspection" performed by City staff, and
submitting a final program 'confbrmance certification will be required prior to issuance of a
Certificate of Occupancyfor the above home.
Further, I hereby acknowledge that plans demonstrating conformance to program criteria or
rating must be submitted -to the Fort Collins Building and Zoning Department in order to obtain art .
"Insulation Inspection", and that failure to use anyof the energy rating programs named above and
