Gentlemen,
Per the request of Mr. Fran Garcia, we have provided the following inspections at the above referenced
address:
1. On November 18, 2002 we inspected the footing form sizes, steel rienforcement and void form
material, prior to the placement of concrete.
2.On November 20, 2002 we inspected the steel reinforcement within the foundation wall forms, prior
to the placement of concrete.
These inspections consisted of visual observations and measurements to confirm that the footing form
widths and depths, steel reinforcement, void form material and foundation wall steel reinforcement
were in general conformance with the approved plans completed by Straight Line Design, Inc., job
#02-335, dated October 10, 2002. Upon completion of each inspection, we have determined that all
requirements pertaining to the inspections have met or exceeded our recommendations.
