Subject: Summary of field changes and quality assurance observations for the residence under
construction at Lot 1 Huntington Hills West Subdivision, Fort Collins. CO.
Mr. Micallef,
This letter summarizes the field changes made to our foundation design, dated July 15", 2003, for
the above referenced residence. Our July 15`h design plan specified reinforced concrete piers and a
(flat plate) 6" thick reinforced structural concrete basement floor- Per your request, Magnum helical
piers were used in lieu of the drilled concrete piers, and a wide flange steel beam framing system
topped with a 3-1/2" thick reinforced concrete deck was used in lieu of the original 6" thick
reinforced concrete basement floor design. We redesigned the foundation with these changes and
provided you with a combination of new plans and verbal recommendations for their installation.
Then, we performed several field observations to verify general compliance to ' our design
recommendations.
In our opinion, the above described field changes did generally meet our design specifications. For
future reference, please find attached the basement floor framing plan which was implemented at the
residence. Other construction quality control observations were performed for the site. These
additional observations performed have been attached as well.
