CONTRACTOR WHO DID A SHODDY JOB. APPLICANT OBTAINED A DEMO PERMIT TO RIP
OUTWORK THAT WAS DONE AND IS OBTAINING THIS PERMIT TO REDO THE WORK. FELIX
IS ALLOWING THE HOMEOWNER TO DO THIS PROJECT SO NO GENERAL CONTRACTOR IS
NEEDED. A HOMEONWER'S AFFIDAVIT WAS OBTAINED. ANY ELECTRICAL, PLUMBING OR
MECHANICAL WORK MUST BE DONE BY A LICENSED CONTRACTOR.

described h rein. I agree to comply with all City ordinances, and State laws associated with such work. I understand that such permit may be revoked in the
event that issuance was based on incorrect or incomplete information. This permit shall become null and void if the work authorized by such permit is not
commenced, suspended, abandoned or not inspected within 180 days from the date of such permit or from the date of the last inspection.
