The following items were not clearly specified in the plans or were not in compliance with the Rules and Regulations Governing the Sanitation of Food Service Establishments in the State of Colorado. These items must be clearly specified in writing or be. modified to be in compliance with the above regulations prior to approval of the submitted plans. The plans are approved at this time. All plans must be approved prior to commencement of construction. 1) All food service equipment must be NSF (National Sanitation Foundation) approved or its equivalent. Custom made equipment and used equipment cannot be approved until it is visually inspected. The following equipment requires visual inspection for approval: shelf, refrigerator, 3-compartment sink, and drainboard extension. The following equipment is not approved at this time due to lack of information and/or lack of NSF approval. Submit make and model numbers and specification sheets on the following equipment: ice bin, and wire racking. 2) Be advised that no appurtenances, such as syrup lines, carbonators, drop -in cold plates, drain lines or bottle holders are permitted in potable ice bins or ice machines or any unit used to store potable ice. 3) This operation is not approved for any type of grease cooking. 4) Restrooms must be provided with mechanical ventilation to the outside air. Submit information indicating if ventilation is provided, and the CFM rating of the exhaust fans. 5) During our July 25, 1997 telephone conversation you indicated that seating for 20 would be provided in the Club. Be advised, the existing restroom fixtures (one unisex room: 1 water closet, 1 sink and 1 urinal) limit club seating to 15 based upon the requirements of the Uniform Plumbing Code. In order to increase seating over 15, separate restroom facilities for men and women must be provided. Also a request for variance to the sections of the Rules and Regulations Governing the Sanitation of Food Service Establishments in the State of Colorado that deal with required toilet facilities (6-401a & 6- 401b) is also- an option. However, the variance request must be approved before seating for over 15 can be provided. Submit information addressing this issue. Vincent's Cigar Club Bar July 28, 1997 Page 2 6)1 A commercial water heater that can achieve a minimum recovery rate of 23 gallons/hour if electric, or has a 6 KW (kilowatt) rating is required. The proposed electric water heater (25 gallons/hour at 100°F rise) is approved. 7) Be advised that the 3-compartment sink tubs must be large enough to submerge all equipment and utensils used in food preparation and food storage. 8) The length of the 3-compartment bar sink's drainboards are 12" on each side. Be advised, drainboards �' must"iie`self draining and -at Ieas(24""in length-During'our"Juiy-25; 1997 telephone conversation you- agreed to provide approved wire shelving and the drainboard extension to make up the needed drainboard space. 9) The mop sink must be provided with hot and cold running water. Provide mop hangers or hooks to hang and air dry mops when they are not in use. 10) During our July 25, 1997 telephone conversation we discussed the bar's menu. The proposed menu includes liquor drinks, beer, wine; and food items from Elliot's Martini Bar menu. Additionally, food will be ordered via intercom by Club members and be served directly to the Club members tables by Elliot's wait staff. No food preparation (besides drinks) or food storage (besides ice) will be conducted on Club premises. The menu is approved at this time. 11) Be advised that all handsinks must be provided with hot and cold running water under pressure through a mixing valve, single service hand towels from a dispenser, and soap. 12) All drain lines from equipment such as: 3-compartment sinks, ice machines, ice bins, drink dispensers, --drink overflowdrains; liquor -or overflow drains; refrigecation/freezer""units,-walk-in compressors -and' cold tables holding ice must be directed to a floor drain or floor sink and be properly air gapped. The 3-,compartment bar sink and the ice bin units are approved to drain indirectly to the bar mop sink as discussed during our July 25, 1997 telephone conversation. 13) Provide information noting the location and size of the grease trap if one is required by the governing waste water authority. The grease trap must be installed out of food preparation areas, and the top of the trap must be installed flush with the finished floor to permit easy cleaning around the equipment. 14) All, plumbing and electrical conduit are to be installed within and behind walls or below floors. Exposed water pipes, sewer lines, or electrical conduit running along walls or floors are not approved. OR ' All drain lines, water lines and electrical conduit which run along the wall must have sufficient space between the lines and the wall to facilitate proper cleaning. Or lines can be boxed in or completely concealed along the wall to facilitate easy cleaning. Exposed pipes, lines, or conduit along the floor is not approved: Vincent's Cigar Club Bar July 28, 1997 Page 4" 23) ' `Please be advised that the following items are required in the establishment at the time of the opening inspection and thereafter: a) stab food thermometer (0 to 220 F) b) food grade plastic gloves and/or finger cots c) approved sanitizer d) test strips for the sanitizer - "—'The above items (#`l, 4; 5;-9; 13, 17b, 21; and 22) can be submitted either in written form of as an addendum to the plans submitted. The written plans submitted are approved at this time. Be advised approval of plans, including the above modifications, does not constitute acceptance of the completed structure. An inspection of the premises must be conducted by this department prior to approval for operation and licensure of the establishment. Failure to follow the recommended changes may prevent or delay the licensure of this establishment. If construction is not commenced within 120 days, any approval.of the plans will be null and void and plans will have to be resubmitted along with a $75.00 plan review fee for a second review. Please notify the department at least 48 hours prior to your opening date so that a final acceptance inspection can be performed. This department's written approval and the receipt of a completed state food service license application and applicable fees is necessary prior to opening. 6775.
