including following the Fort Collins Insulation Guidelines, submitting an insulation certification
form and IIVAC certification form, and obtaining a completed "Insulation Inspection."

Further, I hereby acknowledge that failure to use either of the energy rating programs
named above and meet the respective established minimum criteria for compliance will result in
