Gentlemen:
We have visually observed the footing forms placed at the residence under construction at Lot 65,
Overlook 41h, (1926 Mesaview Lane), a suhdivision of Fort Collins, Colorado. This observation was
performed on April 14, 1999.
It is our opinion that the form widths are in substantial conformance with our Inundation plan, Job
No. 1419-02-22-02. No substantial amounts of loose material were rioted below footing, wall or pad
bearing locations.
phis observation does not, nor was it intended to cover setback requirements. It is the
contractors/owners responsibility to verify that the home has been located within the appropriate
building setbacks.

We have visually observed the wall reinforcing placed in the forms at the residence under construction
at Lot 65, Overlook 4th, (1926 Mesaview Lane), a subdivision of Fort Collins, Colorado. This
observation was performed on April 19, 1999.
It is our opinion that the reinforcement was in general conformance with our foundation plan, Job No-
1419-02-22-02.
