Our records indicate that the new residence you constructed at
the above address received a final inspection on October 28,
1980. Upon reviewing.the inspection record for issuance of the
Certificate of Occupancy, I noted that the foundation inspection
made on July 14, 1980 had not been passed pending a written
statement from Empire Laboratories, Inc. authorizing a foundation
plan change. Notations on our records indicate void material �I
was removed and wall height was raised. No such authorization
has yet been received by this office.
I would urge you to contact Empire and request that they forward
the necessary information to me so that I may release the.certi—
ficate.
