On September 18 and 19, 2000, Weeks & Associates observed the footing sizes, void form
placements, and foundation wall reinforcement steel at the referenced site. Observations
indicate the foundation construction is in substantial conformance with the foundation plan for
the project by Weeks & Associates, Inc., Project No. 20217, dated July 11, 2000.
